摘要

脓毒症(sepsis)是由感染引发的危重疾病,是危重症领域的热点话题。脓毒症发病机制异常复杂,并发症多,其并发症及不良预后与细胞焦亡所触发的炎症失控紧密相连,这一机制不仅有助于揭示脓毒症发生发展的本质,也可为开发新型治疗策略提供理论依据。中药在治疗脓毒症方面具有一定优势,近年来有较多关于中药活性成分调控细胞焦亡防治脓毒症的研究。脓毒症炎症反应时,大量被激活的炎症细胞、炎症因子可触发细胞焦亡,通过Nod样受体蛋白3NLRP3)、半胱氨酸天冬氨酸蛋白酶-1Caspase-1)、Gasdermin DGSDMD)等通路持续合成并分泌白细胞介素-IL-)、白细胞介素-18IL-18)等细胞因子,使炎症反应进一步扩大甚至失控,最终导致器官功能障碍或死亡。中药活性成分能抑制细胞焦亡相关通路的表达而调控细胞焦亡的过程,减少细胞损伤及死亡,改善脓毒症预后。虽然中药活性成分在脓毒症防治中展现出较大潜力,但目前的研究主要以动物实验和体外实验研究为主,今后有必要开展相应的临床研究。本研究对细胞焦亡在脓毒症发病中的作用机制及中药活性成分调控细胞焦亡防治脓毒症的相关研究进展进行综述,旨在为脓毒症的治疗及研究提供思路,进一步发掘临床疗效好、毒副作用小的治疗脓毒症的药物。

Abstract

 Sepsis is a critical illness caused by infection and is a hot topic in the field of critical care. The pathogenesis of sepsis is very complex and it has many complications. Its complications and poor prognosis are closely related to the uncontrolled inflammation triggered by cell pyroptosis. A deep understanding of this mechanism not only helps to reveal the essence of sepsis occurrence and development but also provides theoretical basis for developing new treatment strategies. Traditional Chinese medicine has certain advantages in treating sepsis and in recent years research on the regulation of cell apoptosis by active ingredients in traditional Chinese medicine to prevent and treat sepsis has emerged continuously. During the inflammatory response of sepsis a large number of activated inflammatory cells and cytokines can trigger cell pyroptosis continuously synthesizing and secreting cytokines such as IL-1β and IL-18 through pathways such as NLRP3Caspase-1and GSDMD further expanding or even losing control of the inflammatory response ultimately leading to organ dysfunction or death. The active ingredients of traditional Chinese medicine can inhibit the expression of cell apoptosis related pathways regulate the process of cell apoptosis reduce cell damage and death and improve the prognosis of sepsis. Although the active ingredients of traditional Chinese medicine have shown great potential in the prevention and treatment of sepsis current research mainly focuses on animal experiments and in vitro experiments and it is necessary to conduct corresponding clinical studies in the future. This study reviews the mechanism of cell necrosis in the pathogenesis of sepsis and the related research progress on the regulation of cell necrosis by active ingredients in traditional Chinese medicine for the prevention and treatment of sepsis. The aim is to provide ideas for the treatment and research of sepsis and further explore drugs with good clinical efficacy and minimal toxic side effects for the treatment of sepsis.
