阳气虚弱,无以温煦气化,气血运行不畅,病理产物堆积,化生内毒,伏留胞宫,形成毒结,使得胞宫功能失调,影响宫颈上皮细胞的正常代谢和分化,进而导致宫颈鳞状上皮内病变(SIL)。SIL作为宫颈癌(CC)癌前病变的核心类型,其进展与人乳头瘤病毒(HPV)持续感染、机体免疫失衡相关,若不及时干预可进展为CC,严重威胁女性健康。现代研究揭示,线粒体作为细胞“能量工厂”,其功能障碍是多种疾病病理进程的核心环节。线粒体脱氧核糖核酸(mtDNA)的变异可增强肿瘤细胞的侵袭与转移能力,而线粒体自噬功能障碍及特定基因如线粒体ND1(mtND1)的异常亦与疾病进展密切相关。本研究以中医阳虚毒结理论、线粒体功能、SIL三者的密切关系为切入点,整理相关研究文献,探讨线粒体功能在阳虚形成的SIL中的因果作用,提出线粒体功能障碍将成为SIL阳虚毒结理论研究的关键点,为SIL研究防治提供新视角。研究结果表明,通过调理机体阳虚状态,可有效遏制SIL的进展,而线粒体功能的优化则成为预防SIL向CC转化的新思路。

基于“阳虚毒结”理论的线粒体功能障碍与宫颈鳞状上皮内病变的探讨
The deficiency of yang qi, unable to warm and vaporize, leads to the obstruction of qi and blood circulation. The accumulation of pathological products forms toxins, which generates internal toxins. These toxins linger latently in the uterus, forming toxic aggregations.Consequently, uterine dysfunction occurs, disrupting the normal metabolism and differentiation of cervical epithelial cells, and subsequently leading to cervical squamous intraepithelial lesion (SIL). SIL, as the core type of precancerous lesion of cervical cancer (CC), its progression is associated with persistent infection of human papilloma virus (HPV) and imbalance of the body’s immune system. If not intervened in time, it can progress to CC and seriously threaten women’s health.Modern studies have revealed that mitochondria, as the "energy factory" of cells, its dysfunction is a core aspect of the pathological processes of various diseases. Mutations in mitochondrial deoxyribonucleic acid can enhance the invasive and metastatic ability of tumor cells, while mitochondrial autophagy dysfunction and specific genes such as mitochondrial ND1 abnormalities are also closely related to disease progression.Based on the close relationship between the theory of Yang Deficiency and Toxin Knot, mitochondrial function and SIL in traditional Chinese medicine and the existing literature, this paper explores the potential role of mitochondrial function in the formation of SIL due to yang deficiency,and proposes that mitochondrial dysfunction will become the key point of theoretical research on SIL Yang Deficiency and Toxin Knot, providing a new perspective for the prevention and treatment of SIL. By regulating the state of yang deficiency, it may effectively inhibiting the progression of SIL, and the optimization of mitochondrial function emerges as a novel approach to prevent the transformation of SIL into CC.
