摘要

目的:探讨系统性炎症综合指数(AISI)与重症监护病房(ICU)内急性呼吸窘迫综合征(ARDS)患者短期及长期死亡率之间的关系。方法:基于MIMIC-IV 3.0数据库,对1534ARDS患者开展回顾性队列研究。采用多因素Cox比例风险回归模型评估AISI评分对全因死亡率的影响,并利用限制性立方样条(RCS)探索其潜在的非线性关联。采用X-tile软件确定AISI最佳截断值,将研究对象分为两组,采用Kaplan-Meier生存曲线比较两组间患者的生存差异。开展亚组分析验证研究结果的稳健性。结果:在1534ARDS患者中,高AISI组患者ICU入院30 d死亡率和1年死亡率均高于低AISI组患者。RCS曲线拟合结果显示,AISI30 d死亡率(P=0.021)、1年死亡率(P<0.001)均存在非线性关联。经校正后的Cox回归分析结果表明,与AISI<1528相比,AISI≥1528增加30 d死亡风险(HR=1.24895% CI1.017~1.532)和1年死亡风险(HR=1.33395% CI1.120~1.587)。亚组分析结果表明,合并糖尿病和呋塞米治疗的ARDS患者中,AISI与死亡率之间关联性显著(P<0.05)。结论:AISIICUARDS患者30 d死亡率和1年死亡率的重要预测指标,有望作为一种有效工具,用于早期识别高危患者并指导个体化治疗策略的制定。

Abstract

Objective: To investigate the relationship between the aggregate index of systemic inflammation AISI and short-term and long-term mortality in ICU patients with acute respiratory distress syndrome ARDS.Methods: A retrospective cohort study was conducted on 1534 ARDS patients based on data from the MIMIC-IV 3.0 database.The analysis included multivariable Cox proportional hazards regression to assess the impact of AISI scores on all-cause mortality with restricted cubic splines RCS used for exploring potential non-linear relationships. Optimal AISI cutoff values were determined using X-title software dividing the cohort into two groups. Kaplan-Meier survival curves were compared across these groups supplemented by subgroup analyses to validate findings.Results: Among 1534 ARDS patients the 30-day mortality rate and 1-year mortality rate of patients in the high AISI group after ICU admission were both higher than those in the low AISI group. RCS curve fitting indicated a significant non-linear correlation between AISI and both 30 d P=0.021 and 1 year mortality P<0.001. Cox regression after full adjustment showed that AISI≥1528 was significantly associated with higher risks of mortality at 30 d HR=1.248 95% CI 1.017-1.532 and 1 year HR=1.333 95% CI 1.120-1.587 compared to AISI<1528. Subgroup analyses highlighted significant relevance between AISI and mortality in ARDS patients with diabetes and those treated with furosemide P<0.05.Conclusion: AISI is a significant predictor of both 30 d mortality and 1 year mortality in ARDS patients in ICU. It may serve as an effective tool for identifying high-risk patients early and guiding personalized treatment strategies.
