目的:探讨白内障伴术前角膜散光患者行超声乳化白内障吸除术(Phaco)联合双焦点Toric人工晶体(IOL)植入的视力矫正效果。方法:选取2021年1月至2023年5月在我院治疗的白内障伴角膜散光患者92例122眼,采用信封法将患者分为观察组(46例60眼)和对照组(46例62眼)。观察组给予Phaco联合双焦点Toric IOL植入,对照组给予Phaco联合单焦点Toric IOL植入。观察比较两组患者治疗前后最佳矫正视力(BCVA)、散光度数、角膜厚度、角膜表面曲率及术后不良反应。结果:治疗后3个月,观察组患者近视力BCVA和裸眼近视力分别为(0.07±0.01)LogMAR和(0.14±0.04)LogMAR,均优于对照组(P<0.05)。治疗后3个月,两组患者角膜厚度、角膜前表面扁平子午线方向最小屈光力(FK1)、角膜前表面陡峭子午线方向最大屈光力(FK2)组间和组内比较差异均无统计学意义(P>0.05)。两组患者术后不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:白内障伴术前角膜散光患者行Phaco联合双焦点Toric IOL植入有较好的效果,散光矫正效果以及视力改善较好。

Objective: To explore the vision correction effect of phacoemulsification cataract extraction (Phaco) combined with bifocal Toric intraocular lens (IOL) implantation in cataract patients with preoperative corneal astigmatism.Methods: 92 cases (122 eyes) of cataract patients with corneal astigmatism treated in our hospital from January 2021 to May 2023 were selected and divided into the observation group (46 cases, 60 eyes) and the control group (46 cases, 62 eyes) using the envelope method. The observation group received Phaco combined with bifocal Toric IOL implantation, while the control group received Phaco combined with single focal Toric IOL implantation. The best corrected visual acuity (BCVA), astigmatism degree, corneal thickness, corneal surface curvature, and postoperative adverse reactions of the two groups of patients were observed and compared before and after the treatment.Results: Three months after the treatment, the BCVA of near vision and uncorrected near visual acuity of patients in the observation group were (0.07±0.01) LogMAR and (0.14±0.04) LogMAR, which were better than those in the control group (P<0.05). There was no statistically significant difference between and within the observation group and the control group in terms of corneal thickness, the minimum refractive power (FK1) in the radial direction of the flat anterior surface of the cornea, the maximum refractive power (FK2) in the radial direction of the steep anterior surface of the cornea at 3 months after treatment (P>0.05). There was no statistically significant difference in the total incidence of postoperative adverse reactions between the observation group and the control group (P>0.05).Conclusion: Phaco combined with bifocal Toric IOL implantation has shown good effect in correcting astigmatism and improving vision in cataract patients with preoperative corneal astigmatism.
