摘要

社区获得性肺炎(CAP)因高发病率、高死亡率而成为全球重大公共卫生议题。CAP病原体构成复杂、临床表现多样,致使CAP早期诊断与精准治疗面临较大难题。目前,CAP管理中涉及影像学检查、微生物学检测及生物标志物动态监测等的辅助检查在CAP诊断、治疗调整及预后评估中的价值尚未得到充分释放。尽管有相关诊疗指南可供遵循,但在临床实践中,辅助检查的合理选择、规范操作、结果解读以及系统性疗效评估方面,依然缺乏统一且可操作的标准,进而造成诊疗实践的不一致,对医疗质量提升与资源高效利用形成制约。为CAP辅助检查与疗效评估提供实用规范的参考框架,本课题组邀请各领域专家共同撰写“社区获得性肺炎的辅助检查与疗效评估专家共识”,围绕CAP辅助检查与疗效评估这两大核心环节,将现有最佳证据与临床经验进行系统整合,帮助临床医生更精准地评估病情、优化治疗决策,同时为该领域后续临床实践与研究奠定基础。核心建议包含:基于病情严重程度分层构建影像学检查路径,明确不同临床场景下微生物学检查的适应证及操作规范,规范生物标志物在诊断与治疗监测中的动态应用,搭建整合临床症状、实验室指标及影像学表现的结构化疗效评估体系,同时纳入药学监护与合理用药指导,涵盖抗菌药物的选择、调整、治疗药物监测及不良反应管理;制定治疗失败时的系统性辅助检查与病因排查流程。

Abstract

Community-acquired pneumonia CAP has become a global public health challenge due to its high incidence and mortality rate. The complex composition of its pathogen and diverse clinical manifestations pose significant challenges for early diagnosis and precise treatment of CAP. Currentlythe value of auxiliary examinationsincluding radiological imaging microbiological testing and biomarker dynamic monitoring in CAP diagnosis treatment adjustment and prognostic evaluation has not yet been fully realized.Although there are relevant diagnosis and treatment guidelines to follow in clinical practice in terms of the reasonable selection standardized operation result interpretation and systematic efficacy evaluation of auxiliary examinations there is still a lack of unified and operational standards which leads to inconsistency in diagnosis and treatment practices and restricts the improvement of medical quality and the efficient utilization of resources. To provide a practical and standardized reference framework for CAP auxiliary examinations and efficacy evaluation the research team has invited experts from various fields to jointly write the "Expert Consensus on Auxiliary Examinations and Efficacy Evaluation of Community-Acquired Pneumonia CAP". Focusing on the two core aspects of CAP management auxiliary examinations and efficacy evaluation), this study systematically integrate the best available evidence and clinical experience to help clinicians more accurately assess the condition and optimize treatment decisions. At the same time it lays the foundation for subsequent clinical practice and research in this field. The core recommendations include based on disease severity stratification the imaging examination pathway should be constructed the indications and operation specifications of microbiological examination in different clinical scenarios should be clarified the dynamic application of biomarkers in diagnosis and treatment monitoring should be standardized and a structured efficacy evaluation system integrating clinical symptoms laboratory indicators and imaging manifestations should be established while pharmaceutical care and rational drug use guidance should be included. It covers the selection and adjustment of antibacterial drugs therapeutic drug monitoring and adverse reaction management and establishes a systematic auxiliary examination and etiological screening process for case of treatment failure.
