Abstract:

Objective: To investigate the expression levels and significance of platelet endothelial cell adhesion molecule-1 PECAM-1 and polyligand proteoglycan-1 SDC-1 in elderly patients with severe pneumonia complicated with respiratory failure. Methods: A total of 69 patients with severe pneumonia combined with respiratory failure severe pneumonia and common pneumonia were assigned to respiratory failure group severe pneumonia group and common group respectively.Acute physiology and chronic health status score ⅡAPACHEⅡ), laboratory indexes serum PECAM-1 and SDC-1 levels were compared among all groups. Results: The APACHEⅡ score C-reactive protein and procalcitonin in the group with respiratory failure were significantly higher than those in the severe group and the general group all P < 0.05),and the APACHEⅡ score C-reactive protein and procalcitonin in severe group were significantly higher than those in normal group all P < 0.05.The oxygenation index in the group with respiratory failure were significantly lower than those in the severe group and the general group and serum PECAM-1 SDC-1 level was higher than that in the severe group and the general group all P < 0.05);and the oxygenation index in severe group were significantly lower than those in normal group and serum PECAM-1 SDC-1 level was higher than that in normal group all P < 0.05.In addition serum PECAM-1 and SDC-1 level was moderately positively correlated with the APACHEⅡ score r=0.510 0.585 all P < 0.05), Serum PECAM-1 and SDC-1 levels had a certain predictive value for respiratory failure in patients with severe pneumonia AUC=0.746 0.790.Conclusion: In patients with severe pneumonia complicated with respiratory failure PECAM-1 and SDC-1 are highly expressed and the expression levels of both are closely related to the severity of the patient's disease which has certain predictive value for severe pneumonia complicated with respiratory failure.
