Exploring the Treatment of Pediatric Guillain-Barré Syndrome from the Perspective of Lung Based on the Theory of Flaccidity Syndrome in the Huangdi Neijing

Abstract:

Guillain-Barré syndrome GBS is an acute inflammatory autoimmune disease characterized by demyelination of peripheral nerves and nerve roots. In clinical practice some pediatric GBS patients still experience limb sensory and motor dysfunction after various modern medical treatments with a tendency for disease recurrence. Most GBS patients have a history of preceding infections. Traditional Chinese medicine often classifies this condition under the category of "flaccidity syndrome". As early as the "Huangdi Neijing" period a relatively comprehensive explanation of the etiology and pathogenesis of flaccidity syndrome had already been provided proposing the core pathogenesis of "lung heat scorching" and the treatment principle of "treating flaccidity syndrome by focusing on the Yangming meridian". However based on summarizing the theories of flaccidity syndrome in the "Huangdi Neijing" and the interpretations by later physicians this study argues that "focusing on the Yangming" does not imply exclusivity and merely regulating the spleen and stomach cannot fully address flaccidity syndrome. Considering the etiology of pediatric GBS the physiological characteristics of "excess of Yang in children" and the functional connection between the lung and spleen this study has identified "external pathogens invading the lung stagnating and transforming into heat heat scorching the lung " as the core pathogenesis of this disease. Phlegm-dampness and blood stasis are significant pathological byproducts. The overall treatment principle should be to address both symptoms and root causes tonify the deficiency and purge the excess with a focus on treating the lung. Methods such as dispersing wind promoting lung function clearing heat and unblocking collaterals are employed. The modified formula of Ma Xing Yi Gan decoction combined with Gui Zhi Jia Ge Gen decoction has been clinically validated as effective offering new insights for clinical diagnosis and treatment.
