The Application Value of Kuntujianzhong Therapy Based on Theory of Visceral Manifestation in the Treatment of ICU-ENAD

Abstract:

Objective: To explore the application value of kuntujianzhong therapy based on theory of visceral manifestation in the treatment of enteral nutrition associated diarrhea ENAD in intensive care unit ICU.Methods: 120 patients with ENAD after enteral nutrition EN from ICU of our hospital were enrolled between July 2021 and June 2024. According to random number table method the patients were divided into the observation group n=60 and the control group n=60. On basis of routine EN treatment the control group was treated with probiotic live combined bacillus subtilis and enterococcus faecium enteric-coated capsules combined with montmorillonite powder while the observation group was treated with Kuntujianzhong therapy under the theory of Visceral Manifestation on basis of the control group for 5 d. The clinical curative effect in the two groups was observed. The scores of TCM syndromes change of stool property Hart diarrhea scale), diarrhea degree diarrhea degree scale), diarrhea duration nutritional indexes serum albumin prealbumin), gastrointestinal quality of life index GIQLI and occurrence of incontinence-associated dermatitis were compared between the two groups before and after intervention.Results: After intervention the reduction of scores of TCM syndromes loose stool pale complexion limbs coldness), Hart diarrhea scale and diarrhea degree in the observation group were greater than those in the control group P<0.05); diarrhea duration was shorter than that in the control group P<0.05); levels of serum albumin and prealbumin and GIQLI score were higher than those in the control group P<0.05); total response rate was higher than that in the control group 96.67% vs. 80.00%P<0.05), and incidence of incontinence-associated dermatitis was lower than that in the control group 0.00% vs. 10.00%P<0.05.Conclusion: The application value of Kuntujianzhong therapy based on theory of Visceral Manifestation is good in ICU-ENAD patients which can effectively regulate spleen-stomach function improve clinical symptoms shorten diarrhea duration improve clinical curative effect reduce incontinence-associated dermatitis and improve quality of life.
