Objective： To explore and summarize Xia Yongliang’s differential diagnosis and treatment approach for cervical lymph node swelling based on the "Six Stagnations" theory. Methods： Clinical cases of cervical lymph node swelling treated by Xia Yongliang in outpatient settings were collected and analyzed to summarize his therapeutic experience and medication characteristics. A typical case was examined in detail to further illustrate his clinical insights. Results： Based on clinical practice， Xia Yongliang derived a treatment method for cervical lymph node swelling from Zhu Danxi’s "Six Stagnations" theory. The six stagnations—qi， blood， phlegm， fire， dampness， and food*—can occur individually or in combination， leading to disease. The key to clinical diagnosis lies in discerning the relationships among these stagnations and determining their "severity and urgency." Treatment should focus on regulating qi movement and resolving stagnation as the foundation， with tailored prescriptions based on the specific "severity and urgency" of each case. In the presented case， Xia Yongliang diagnosed the condition as internal accumulation of damp-heat and phlegm-turbidity obstruction， adopting a treatment strategy of clearing heat， resolving dampness， dissolving phlegm， and relieving stagnation. The modified Sanren Tang （Three-Nut Decoction） combined with Erchen Tang （Two Matured Substances Decoction） and Shengjiang San （Upbearing and Downbearing Powder） yielded significant therapeutic effects. Conclusion： Xia Yongliang’s approach to cervical lymph node swelling， rooted in Zhu Danxi’s "Six Stagnations" theory， integrates practical clinical considerations. By flexibly applying classical and contemporary formulas tailored to the specific etiology and pathogenesis of each case， he achieves both symptomatic and root-level treatment. His clinical experience offers valuable insights for fellow practitioners.
