      目的:研究胃黏膜癌变过程中幽门螺杆菌(Hp)感染与CD44表达的关系。方法:用免疫组化SP法检测慢性非萎缩性胃炎、慢性萎缩性胃炎、胃黏膜肠上皮化生、胃黏膜不典型增生及胃癌组织各25例中CD44蛋白的表达,并检测患者Hp感染情况。结果:5种疾病胃黏膜组织中Hp感染平均阳性率为50.4%,CD44蛋白表达平均总阳性率为42.4%。随着胃黏膜癌变过程的加重,Hp感染及CD44的阳性表达随之升高,差异有统计学意义(P<0.05);慢性非萎缩性胃炎、胃黏膜肠上皮化生、胃黏膜不典型增生及胃癌组织Hp感染率与CD44的阳性率均呈正相关,相关系数分别为0.408、0.441、0.445、0.831,差异有统计学意义(P<0.05)。结论:Hp感染是胃癌发生发展的重要因素,胃黏膜病变过程可诱导有细胞增殖特性的CD44蛋白的表达。CD44可作为胃癌前病变早期生物学信号。
英文摘要:
      Objective: To study the relationship between helicobacter pylori (Hp) infection and CD44 expression in carcinogenesis of gastric mucosa. Methods: Immunohistochemical SP method was used to detect the expression of CD44 in chronic non-atrophic gastritis,chronic atrophic gastritis,intestinal metaplasia, dysplastic hyperplasia and gastric carcinoma,and Hp infection was detected in 25 patients of each tissue. Results: The positive rate of Hp infection during gastric mucosa was 50.4%, and the average positive rate of CD44 protein expression was 42.4% in all the 5 diseases. Hp infection and the positive expression of CD44 increased along with the exacerbation of gastric carcinogenesis, the difference was statistically significant(P<0.05). Hp infection rate and the positive rate of CD44 in chronic non-atrophic gastritis,gastric mucosal intestinal metaplasia,gastric dysplasia and gastric cancer tissue were positively correlated,the correlation coefficients were 0.408,0.441,0.445,0.831, and the difference was statistically significant(P<0.05).Conclusion:Hp infection is an important factor in the development of gastric cancer. During the course of gastric mucosal, it induces the expression of CD44 protein with cell proliferation characteristics.CD44 can serve as an early biological signal for precancerous lesions.
