口咽通气管在无痛人工流产术中的临床应用

Oropharyngeal Ventilation Tube in Painless Poured Technique the Clinical Application

摘要: 目的:观察放置口咽通气管在静脉麻醉下人工流产术(无痛人流)患者中的临床应用。方法:600例无痛人流患者随机分为两组,309例患者(观察组)以丙泊酚复合芬太尼静脉麻醉后经口放置口咽通气管面罩吸氧下行无痛人流术,对同期291例患者(对照组)采取同样麻醉手术方法但不放置口咽通气管。观察和记录两组患者麻醉手术过程中的麻醉效果、反应性症状,包括人流综合征、体动、鼾声呼吸、呼吸困难(一过性血氧饱和度下降至90%需采取托下颌或人工辅助吸氧处理)及各时段的心率、血氧饱和度等主要生命体征变化情况。结果:两组患者一般资料、麻醉效果比较无差异且所有患者均未出现人流综合征;术中观察组和对照组分别有38例和40例轻微体动;观察组没有病例出现鼾声呼吸,仅有8例出现一过性呼吸困难,对照组有80例出现鼾声呼吸,56例出现一过性呼吸困难,两组间比较有统计学意义(P<0.01);两组各时段HR、SpO2比较没有统计意义(P>0.05)。结论:放置口咽通气管能有效缓解无痛人流手术中静脉麻醉所致舌后坠引起的上呼吸道梗阻,保持良好通气,提高手术的安全性。

Abstract: Objective:Observe placed oropharyngeal ventilation tube under intravenous anesthesia abortion profilometry(painless poured) patients clinical application.Methods:600 people painlessly cases were randomly divided into two groups,309 patients(the observation group) with acryloxyethyl tabor phenolic compound fentanyl after intravenous anesthesia by mouth placed oropharyngeal ventilation tube mask oxygen flow of transmutation,descending painless same 291 patients(control group) to take a similar anesthesia surgical method but not placed []oropharyngeal ventilation tube.Observe and record two groups of patients in the process of anesthesia anaesthetic effect,reaction symptoms,including poured syndrome,ody dynamic and snoring breathing,difficulty breathing(a transient proportional desaturation to 90% take torsten jaw or artificial auxiliary oxygen treatment) and in each period of heart rate,blood oxygen saturation and other major life changes signs.Results:Two groups of patients general data,anaesthetic effect no difference compared with all of the patients did not appear poured syndrome;Intraoperative trial group and control group there were 38 cases and 40 patients with mild body move;The observation group without interrupion breathing,only cases appeared in 8 cases occurred sex difficulty breathing,group compared with 80 cases snoring breathing,56 cases occurred sex difficulty breathing,comparison between the two groups was statistically significant(P<0.01),Two groups of each period HR,compare SpO2 was not statistically significant(P>0.05).Conclusion:Placed oropharyngeal ventilation tube can effectively relieve people surgery painlessly caused by intravenous anesthesia tongue in the upper respiratory tract obstruction caused after falling,improve,maintain good ventilation of the safety of the operation.
