The Influence of Combined Epidural-spinal Anesthesia on Anesthesia Effect and Hemodynamics in Hysterectomy

摘要: 目的:探讨腰硬联合麻醉对子宫切除术患者麻醉效果及血液动力学的影响。方法:将100例子宫切切除术的患者分为观察组和对照组,对照组采用硬膜外麻醉,观察组采用腰硬联合麻醉,比较两组在麻醉前(T0)、气管插管(T1)、切皮(T2)、切除子宫时(T3)和拔除气管导管(T4)时刻的收缩压(SPB)、舒张压(DPB)、心率(HR)的变化、麻醉起效时间、麻醉效果和不良反应。结果:两组患者的SPB、DPB、HR在T0时无显著性差异(P>0.05)。两组在T1、T2、T3和T4时刻的SPB、DPB显著低于T0(P<0.05)。观察组在T1、T2、T3和T4的SPB、DPB、HR与对照组相同时刻点的三项指标有显著性差异(P<0.05)。观察组的麻醉起效时间、麻醉效果、不良反应显著优于对照组(P<0.05)。结论:腰硬联合麻醉用于子宫切除术起效快,用药量少,不良反应少,血液动力学更稳定。

Abstract: Objective:To investigate The Influence of combined epidural-spinal anesthesia on anesthesia effect and hemodynamics in hysterectomy.Methods:100 cases of uterine earnestly operation were divided into the observation group and the control group,the control group was given epidural anesthesia,the observation group was treated with combined spinal-epidural anesthesia,the two groups were compared with the systolic blood pressure(SPB),diastolic blood pressure(DPB),heart rate(HR) changes before anesthesia(T0),endotracheal intubation(T1) skin incision(T2),excision of uterine and tracheal extubation time(T3) catheter(T4) moments of onset time of anesthesia,anesthetic effect and adverse reaction.Results:SPB,DPB,HR in T0 in two groups had no significant difference(P>0.05) time SPB,DPB in T1,T2,T3 and T4 in two groups was lower than that in T0(P<0.05).SPB,DPB,HR in T1,T2,T3 and T4 in the observation group and the control group at the same time points had significant difference(P<0.05).The onset time of anesthesia,anesthetic effects,adverse reactions in the observation group were significantly better than the control group(P<0.05).Conclusion:Combined spinal and epidural anesthesia for hysterectomy has the advantages of quick response,less dosage,fewer adverse reactions,hemodynamic stability.
