双水平无创正压通气配合低分子肝素对慢性阻塞性肺病患者肺功能及氧代谢的影响

Noninvasive Positive Pressure Ventilation Effect with Low Molecular Weight Heparin on Chronic Obstructive Pulmonary Disease with Type Ⅱ Respiratory Failure in Patients with Lung Function and Oxygen Metabolism

摘要: 【摘要】目的:分析双水平无创正压通气配合低分子肝素对慢性阻塞性肺病(COPD)合并Ⅱ型呼吸衰竭患者的肺功能及氧代谢的影响效果。方法:以80例COPD合并Ⅱ型呼吸衰竭患者为研究对象,将患者随机平均分为观察组和对照组,对照组患者采用低分子肝素治疗,观察组患者在此基础上使用双水平无创正压通气治疗,对比治疗前后两组患者肺功能及氧代谢变化情况。结果:治疗后3d,观察组患者的FEV1.0、PEF25、PEF50及TLC水平显著高于治疗前,并且显著高于对照组患者治疗后各项指标水平,治疗后两组患者各项指标比较差异有统计学意义(P<0.05)。治疗后的观察组患者的SvO2、CaO2、水平显著高于对照组,ERO2、VO2水平低于对照组,SvO2、CaO2、ERO2、VO2水平差异有统计学意义(P<0.05)。结论:双水平无创正压通气配合低分子肝素提升COPD合并Ⅱ型呼吸衰竭患者肺功能及氧代谢的效果更显著。

Abstract: 【ABSTRACT】Objective: To analyze the effect of non invasive positive pressure ventilation combined with low molecular weight heparin on pulmonary function and oxygen metabolism in patients with chronic obstructive pulmonary disease complicated with type Ⅱ respiratory failure.Methods:In 80 cases of COPD patients with type Ⅱ respiratory failure patients as the object of study, patients were randomly divided into observation group and control group, control group were treated with low molecular heparin treatment, the observation group patients based on the use of bi level noninvasive positive pressure ventilation in the treatment, before and after the treatment were compared two groups of patients with pulmonary function and oxygen metabolism changes.Results:After 3 days of treatment,treatment observation group patients FEV1.0, PEF25, pef50 and TLC level was significantly higher than that before treatment, and were significantly higher than those of control group patients after the levels of the indicators, after treatment, two groups of patients with various indicators difference difference has statistical significance (P<0.05).After treatment, the observation group of patients with SvO2, CaO2, levels were significantly higher than the control group, ERO2, VO2 levels were lower than the control group, SvO2, CaO2, ERO2, VO2 levels were statistically significant (P<0.05). Conclusion:The double level of non invasive positive pressure ventilation combined with low molecular weight heparin (LMWH) enhance COPD patients with type Ⅱ respiratory failure patients with lung function and oxygen metabolism has a more significant effect, worthy of application in the treatment of COPD with type Ⅱ respiratory failure patients.
