血浆B型脑钠肽与急性心肌梗死心功能的预测

Prediction of Plasma B Type Natriuretic Peptide and Cardiac Function in Patients with Acute Myocardial Infarction

摘要: 【摘要】目的:探讨血浆B型脑钠肽(BNP)对急性心肌梗死心功能的预测。方法:选取入住本院的77例急性心肌梗死住院患者,回顾性分析患者基本资料,并进行心脏超声检查测量左室射血分数(LVEF),随访3个月后,再次检测BNP浓度及心脏超声检查,记录患者死亡情况。结果:按照Killip分级将急性心肌梗死患者分为4组,4组患者急性期血浆BNP水平逐级升高,差异显著P<0.05),且BNP水平与Killip分级呈正相关(r=0.937,P<0.01);患者BNP水平与LVEF呈负相关(r=-0.916,P<0.01);观察期间共死亡10例,比较死亡组与存活组的BNP,死亡组患者急性期的血浆BNP、Killip分级显著高于存活组(P<0.01),死亡组LVEF显著低于存活组(P<0.01)。结论:血浆B型脑钠肽对急性心肌梗死患者的心功能有很好的预测作用。

Abstract: 【ABSTRACT】Objective: To explore the prediction function on cardiac function of plasma B type natriuretic peptide in acute myocardial infarction.Methods:Select 77 cases of acute myocardial infarction in our hospital,record the name,sex,age,smoking and drinking history,basic diseases,Killip classification of cardiac function of the patients,measure the plasma BNP level,and measure the left ventricular ejection fraction (LVEF) by the cardiac ultrasound in 24 hours. After 3 months of follow-up,detect the concentration of BNP and do the cardiac ultrasound again,record the cases of death. Results:The patients were divided into 4 groups according to Killip classification of acute myocardial infarction, the BNP levels gradually increased and the difference is significant (P<0.05).The BNP level and Killip classification has positive correlation(r=0.937,P<0.01).The BNP level and LVEF has negative correlation(r=-0.916,P<0.01).10 patients died during the observation period,the BNP level and Killip classification in the acute phase of death group were Significantly higher than that of the survival group.The LVEF of death group were Significantly lower than that of the survival group(P<0.01). Conclusion:The BNP has a good prediction function on cardiac function in acute myocardial infarction.
