脑膜中动脉创伤性假性动脉瘤破裂致脑内出血的临床分析

摘要: 摘要:目的:研究分析脑膜中动脉创伤性假性动脉瘤破裂致脑内出血的临床特点和治疗。方法:回顾性分析我院于2011年1月~2014年2月收治的60例脑膜中动脉创伤性假性动脉瘤破裂致脑内出血患者的病历资料,通过分析患者的临床表现和影像学特点,并结合手术方法进行分析。结果:60例患者均进行翼点入路手术,术后患者病理提示,没有正常的血管壁成分,和假性动脉瘤的特点符合。患者出院时,结合脑卒中患者日常的生活活动能力测评,其中能力重度障碍者有10例,生活能力轻度障碍者有30例,生活自理者有2例。所有患者进行心脏血管造影检查,患者动脉瘤全部消失,同时患者有着较好的侧支循环代偿和通畅的载瘤动脉。结论:脑膜中动脉创伤性假性动脉瘤破裂致脑内出血患者术前诊断较为困难,同时采取手术夹闭和塑形较为困难,在术前更要对颅内血管的代偿充盈情况进行了解,并借助于现代化显微镜手术进行治疗,将会有着较好的手术效果。

Abstract: Abstract: objective: to study the analysis of traumatic pseudoaneurysms middle meningeal artery rupture caused by the clinical characteristics and treatment of cerebral hemorrhage.Methods: a retrospective analysis in January 2011 to February 2014 were 60 cases of traumatic pseudoaneurysms middle meningeal artery rupture caused by the medical records of patients with cerebral hemorrhage, through the analysis of the clinical manifestations and imaging features of patients, and combined with surgical method for analysis.Results: 60 patients were pterion approach surgery, patients with postoperative pathological hint, no normal vessel wall components, and the characteristics of pseudoaneurysms.Patients discharged from hospital, combined with cerebral stroke patients daily life activities ability assessment, including ability to severe disabilities in 10 cases, life ability has 30 cases of mild disabilities, self-reliance in 2 cases.All patients with cardiac angiography, patients with aneurysms disappear entirely, at the same time patients have better compensatory collateral circulation and accessibility of the parent artery.Conclusion: traumatic pseudoaneurysms middle meningeal artery rupture caused by cerebral hemorrhage patients with preoperative diagnosis is difficult, at the same time to clip and shaping is difficult, more to compensate for intracranial vascular filling in preoperative condition for understanding, and with the aid of modern microscope surgical treatment, will have a good operation effect.
