超声弹性应变率比值法对乳腺结节良恶性的鉴别诊断价值

Differential Diagnosis Value of Benign and Malignant Breast Nodules by Ultrasonic Strain Rate Ratio Method

摘要: 【摘要】目的:探讨超声弹性应变率比值法(SR)在乳腺良恶性病变鉴别诊断中的应用价值。方法:对我院收治的68例乳腺患者(72个乳腺结节)进行超声弹性成像,与病理结果对照,应用ROC曲线确定SR值的最佳临界值。结果:经组织病理学确认的72个乳腺结节有44个结节为良性,28个恶性,行弹性超声成像结果进行分析,恶性结节SR为(0.35±0.14),良性结节的SR为(1.06±0.68),二者比较差异有统计学意义(P<0.01),SR的最佳临界值为0.40,以SR<0.4作为诊断恶性结节的标准,其敏感性90.2%,特异性89.3%,准确性90.6%。根据该值构建ROC曲线下面积为0.912,有较高的诊断准确性。结论:SR对判断乳腺结节良恶性的准确率较高,有良好的应用价值。

Abstract: 【ABSTRACT】Objective: To evaluate the value of quantitative analysis in the benign and malignant breast nodules by ultrasonic elastosonography strain ratio(SR).Methods:68 cases of breast patients (72 breast nodules) in our hospital were included in the study.All of the breast nodules were examined by ultrasonic elastography from which the SR were derived,compared with the pathological results,and the ROC curve was used to identify the best threshold for differential diagnosis of breast nodules.Results: Through the histological pathology confirmation of 72 breast nodules,44 nodules were benign and 28 nodules were malignant. Analyzed the results by ultrasonic elastography,The SR of malignant nodules were (0.35±0.14),benign nodules were (1.06±0.68),P<0.01,It showed that the optimal operating point was 0.40,with high sensitivity(90.2%),specificity(89.3%)and accuracy (90.6%),The area under the ROC curve was 0.912.Conclusion:SR has a high accuracy to judge the benign and malignant breast nodules and it has good application values.
