Timing and Value of Sequential Noninvasive Ventilation in Patients with Acute Respiratory Distress Syndrome after Early Extubation

摘要: 【摘要】目的:探讨急性呼吸窘迫综合征患者早期拔除气管插管后序贯无创通气的效果。方法:选取2013年10月~2015年12月我院收治的100例急性呼吸窘迫综合征患者随机分成对照组和观察组,比较相关指标及效果。结果:两组患者治疗后的呼吸情况、心率以及氧合指数、PaCO2、MAP比较,观察组显著优于对照组,差异有统计学意义(P<0.05)。观察组有创机械通气时间、总机械通气时间、住院时间显著低于对照组(P<0.05),且其VAP发生率、死亡率较对照组均明显更低(P<0.05)。结论:在急性呼吸窘迫综合征患者早期拔除气管插管后进行序贯无创通气治疗,最佳时机为氧合指数200~250mmHg。能有效改善呼吸状况,缩短住院时间,降低死亡率。

Abstract: 【ABSTRACT】Objective: To investigate the effect of sequential noninvasive ventilation on early extubation in patients with acute respiratory distress syndrome.Methods:100 patients with acute respiratory distress syndrome in our hospital from Oct 2013 to Dec 2015,were divided into the control group and the observation group randomly.compared Indicators and effects with two groups.Results:After treatment,compared the Breathing,heart rate,and oxygenation index,PaO2,MAP with the two groups,the observation group was better than the control group significantly,the difference was statistically significant (P<0.05).compared the Invasive mechanical ventilation time,the total duration of mechanical ventilation,length of hospital stay with the two groups,the observation group was better than the control group significantly,the difference was statistically significant (P<0.05).compared the VAP incidence,mortality with the two groups,the observation group was lower than the control group significantly,the difference was statistically significant (P<0.05).Conclusion:In the patients with acute respiratory distress syndrome,the patients were treated with sequential non-invasive ventilation after early extubation.The best time was the oxygenation index 200~250mmHg.Can effectively improve the breathing condition,shorten the length of stay,reduce the mortality rate.
