一氧化碳中毒患者院前高流量给氧气联合高压氧仓对迟发性脑病和细胞因子的影响

Effects of High Flow Oxygen Combined with Hyperbaric Oxygen on the Delayed Encephalopathy in Patients with Carbon Monoxide Poisoning

摘要: 目的:探讨高流量给氧联合高压氧仓对CO中毒患者发生迟发性脑病的影响。 方法:选取于2016年1月至2017年1月我院急诊收治的66例急性CO中毒患者作为研究对象,采用随机数字表法将患者分为研究组(基础急救+高流量给氧+高压氧)和对照组(基础急救+高流量给氧),观察两组患者治愈、迟发型脑病发生和昏迷情况,于入院1、15、30d采集外周血,观察血清白介素-4(IL-4)、白介素-10(IL-10)、结缔组织生长因子(CTGF)、转化生长因子-β1(TGF-β1)和γ-干扰素(IFN-γ1)水平。结果:研究组患者痊愈率75.76%,高于对照组的48.48%,迟发性脑病发生率6.06%,低于对照组的24.24%,差异有统计学意义(P<0.05);研究组昏迷时间<4h者10例,高于对照组3例,差异有统计学意义(χ2=4.69,P<0.05);两组患者在入院1d时血清IL-10、CTGF、TGF-β1和IFN-γ1水平升高,血清IL-4水平降低,对照组患者各项指标水平均显著低于研究组,差异有统计学意义(P<0.05)。 结论:高流量给氧联合高压氧能够有效预防CO中毒患者迟发性脑病的出现,提高治愈率。

Abstract: Objective:To investigate the effect of high flow oxygen combined with hyperbaric oxygen on delayed encephalopathy in patients with carbon monoxide poisoning. Methods: 66 patients with acute carbon monoxide poisoning were enrolled in our hospital from January 2016 to January 2017,which were random allocated into the study group (basal first aid + high flow oxygen + hyperbaric oxygen) and control group (basal first aid + high flow oxygen). Cure, delayed encephalopathy and coma were observed. On the 1st, 15th and 30th day of admission, the levels of serum IL-4, IL-10, CTGF, TGF-β1 and IFN-γ1 were observed. Results: The cure rate of the study group was 75.76%, which was lower than that of the control group (48.48%), and the incidence of delayed encephalopathy was 6.06%, which was lower than that of the control group (24.24%), the differences were statistically significant (P<0.05). The cases of coma time<4 h were 10 cases in study group,which was more than that of control group(3 cases), the difference was statistically significant (χ2=4.69, P<0.05). The levels of IL-10, CTGF, TGF-β1 and IFN-γ1 in the two groups increased in the 1st day, the lvevl of IL-4 decreased. The level of each index in control group were significantly inferior to the study group, the difference was statistically significant(P<0.05). Conclusion: High flow oxygen combined with hyperbaric oxygen can effectively prevent delayed encephalopathy, improve the cure rate.
