CysC及炎性因子在高血压肾病早期诊断评估中的价值研究

The Value of Cys C and Inflammatory Factors in the Early Diagnosis of Hypertensive Nephropathy

摘要: 目的:探讨血清胱抑素C(Cys C)、炎性因子[C-反应蛋白(CRP)、白介素-1β(IL-1β)、白介素-10(IL-10)、转化生长因子-β1(TGF-β1)]5项指标联合检测在高血压肾病早期诊断及病情评估中的价值。方法:选取60例高血压肾病患者作为高血压肾病组和50例健康对照者作为对照组,检测两组人群血清中Cys C、CRP、IL-1β、TGF-β1、IL-10的水平,另根据Cys C水平将高血压肾病组分为Cys C≥1. 2 mg·L-1组和Cys C<1. 2 mg·L-1组,两组比较CRP、IL-1β、TGF-β1、IL-10水平,使用 SPSS 19.0统计软件对结果进行分析,采用Spearman进行单因素相关分析。结果:高血压肾病组患者血清Cys C、CRP、IL-1β 水平均高于对照组(P<0.05);而IL-10、TGF-β1均低于对照组(P<0.05);Cys C≥1. 2 mg·L-1组CRP、IL-1β水平明显高于Cys C<1.2 mg·L-1组,TGF-β1、IL-10水平低于Cys C<1.2 mg·L-1组,差异具有统计学意义 (P<0. 05)。Spearman分析结果显示,Cys C 与CRP、IL-1β均呈现显著正相关(P<0. 05),与TGF-β1、IL-10呈现显著负相关(P<0. 05)。结论:Cys C在反映高血压肾病患者肾功能变化的同时亦反映了体内炎症状态,可用于判断高血压肾病的病情严重程度。Cys C及炎性因子联合测定可判断肾功能损害的情况,为临床对疾病的早期诊断及治疗提供实验室依据。

Abstract: Objective: To explore the value of combined detection of five indicators [cystatin C(Cys C) and inflammatory factor:C-reactive protein (CRP), interleukin-1β (IL-1β),interleukin-10 (IL-10),transforming growth factor-β1(TGF-β1)] in the early diagnosis and evaluation of hypertension nephropathy.Methods: Selected 60 cases of hypertensive nephropathy patients as the hypertensive nephropathy group,and 50 healthy people as the control group.Detected the levels of Cys C, CRP, IL-1β, TGF-β1 and IL-10 in the serum of the two groups. Hypertensive nephropathy group was divided into Cys C≥1.2 mg·L-1 grpup and Cys C<1.2 mg·L-1 group according the Cys C level. Then the groups were detected the levels of CRP, IL-1β,TGF-β1 and IL-10.The results were analyzed by SPSS 19.0, and Spearman was used for single factor correlation analysis.Results: Compared with the control group, the levels of Cys C,CRP and IL-1β in the hypertensive nephropathy group were higher, while the levels of TGF-β1 and IL-10 were lower (P<0.05). The levels of CRP and IL-1β in Cys C≥1.2 mg·L-1 group were significantly higher than Cys C<1.2 mg·L-1 group, and the levels of TGF-β1 and IL-10 were significantly lower than Cys C<1.2 mg·L-1 group(P<0.05).Spearman analysis showed that Cys C was positively correlated with CRP and IL-1β (P<0.05), and negatively correlated with TGF-β1 and IL-10 (P<0.05).Conclusion: Cys C can not only respond to changes in renal function of hypertension nephropathy patients,but also can reflect the body′s inflammatory response state, it is important to determine the severity of hypertensive nephropathy.Cys C combined with inflammatory factor can make a greater extentdetermine the damage of renal function,and it can play a role in clinical early diagnosis, early treatment and early rehabilitation.
