产前彩超与MRI诊断植入型凶险性前置胎盘的效果分析

Diagnostic Effect Analysis of Antenatal Color Doppler Ultrasound and MRI on Diagnosing Invasively Pernicious Placenta Previa

摘要: 目的: 探讨产前彩超与核磁共振(MRI)诊断植入型凶险性前置胎盘的效果。方法:对72例凶险性前置胎盘病例产前分别采用彩超、MRI进行筛查诊断,术后进行病理检查确诊;计算两种检查方法的灵敏度、特异度、漏诊率、误诊率、诊断符合率,并对以上指标进行比较。结果: 产前MRI诊断植入型前置胎盘的灵敏度、特异度、漏诊率、误诊率、诊断符合率分别为84.6%、84.8%、15.4%、15.2%及84.7%,而产前彩超分别为74.4%、90.9%、25.6%、9.1%及81.9%;产前MRI诊断植入型前置胎盘的灵敏度、诊断符合率均高于产前彩超,但差异均无统计学意义(P>0.05)。结论: 产前彩超、MRI对植入型凶险性前置胎盘均有较好的诊断效果,但MRI检查能够明确胎儿、胎盘、子宫肌层之间的关系,对于彩超无法确诊而有条件的医院,可以考虑采用MRI复查。

Abstract: Objective:To explore the diagnostic effect of antenatal color doppler ultrasound and MRI on diagnosing invasively pernicious placenta previa. Methods: 72 cases of pernicious placenta previa were screened by antenatal color doppler ultrasound and MRI, and definitely diagnosed by pathological examination. The indexes of sensitivity, specificity, missed diagnosis rate, misdiagnosis rate and diagnostic accordance rate were calculated and compared between antenatal color doppler ultrasound and MRI. Results: The sensitivity, specificity, missed diagnosis rate, misdiagnosis rate and diagnostic accordance rate in the MRI were respectively 84.6%, 84.8%, 15.4%, 15.2% and 84.7%, and 74.4%, 90.9%, 25.6%, 9.1% and 81.9% in the color doppler ultrasound. There were no statistical significance about sensitivity and diagnostic accordance rate between antenatal color doppler ultrasound and MRI (P>0.05). Conclusion: Antenatal color doppler ultrasound and MRI both have good diagnosing effect on diagnosing invasively pernicious placenta previa. But MRI examination can be clear on the relationship of fetus, placenta and myometrium. MRI examination can be chosen, if can′t be definitely diagnosed by antenatal color doppler ultrasound and have MRI equipment in your hospital.
