摘要/Abstract

摘要： 目的：探讨记忆合金环抱器内固定修复连枷胸的临床效果及对患者血气指标的影响。方法：将我院收治的68例连枷胸患者随机分为对照组与观察组，对照组采用保守治疗，观察组采用记忆合金环抱器内固定治疗。比较两组治疗后住院时间、机械通气时间、并发症发生情况、疼痛程度、血气指标改善情况、肺功能改善情况。结果：观察组住院时间、机械通气时间短于对照组（P＜0.05），肺部感染发生率低于对照组（P＜0.05）。观察组无痛比例高于对照组（P＜0.05）。两组治疗后动脉血氧分压（PaO2）高于治疗前，动脉血二氧化碳分压（PaCO2）低于治疗前，观察组改善效果优于对照组（P＜0.05）。观察组治疗后最大肺活量（FVC）、第1秒用力呼气量（FEV1）、肺总容量（TLC）、呼气高峰流量（PEFR）均高于对照组（P＜0.05）。结论：采用记忆合金环抱器内固定修复连枷胸，临床效果显著，可缩短住院时间及机械通气时间，并发症发生率低，治疗后血气分析指标、肺功能指标、疼痛程度改善明显。

Abstract: Objective： To investigate the clinical effect of internal fixation of flail chest with memory alloy embracing fixator and its effect on blood gas indexes. Methods： Sixty-eight cases of flail chest patients admitted to our hospital were randomly divided into the control group and the observation group. The control group received conservative treatment. The observation group was treated with memory alloy embracing internal fixation. The hospitalization time， mechanical ventilation time and complication incidence were compared between the two groups after treatment. The degree of pain after treatment， the improvement of blood gas indexes and the improvement of pulmonary function were compared. Results： The hospitalization time， mechanical ventilation time in observation group were shorter than those in control group (P<0.05). The incidence of pulmonary infection in observation group was lower than that of control group (P<0.05). The proportion of painless in observation group was significantly higher than the control group (P<0.05). After treatment， PaO2 was significantly higher than before treatment， PaCO2 was significantly lower than before treatment， and the observation group improved better than the control group (P<0.05). The FVC， FEV1， TLC and PEFR in the observation group were significantly higher than those in the control group (P<0.05). Conclusion： Internal fixation of flail chest with memory alloy embracing fixator has significant clinical effect， which can shorten the length of hospital stay and time of mechanical ventilation. The complication rate is low. Blood gas analysis indexes， lung function indexes and pain degree after treatment are obviously improved.
