发育不良患儿低白蛋白血症与其免疫水平和医源性感染的相关性

Study on the Correlation between Hypoalbuminemia and Immune Level and Iatrogenic Infection in Children with Developmental Dysplasia

摘要: 目的:探究发育不良患儿低白蛋白血症与其免疫水平和医源性感染的相关性。方法:根据患者的临床症状,随机选取我院就诊的100例发育不良的低白蛋白血症患儿归入低白蛋白血症组(n=100);随机选取同期在我院行体检、身体机能近似的100例身体发育健康患儿归入对照组(n=100)。通过临床数据收集,对于患儿的身体状况、血浆白蛋白水平、血浆白蛋白代谢动力学参数、免疫球蛋白表达水平和医源性感染发生率等指标进行评价。结果:相比于对照组,低白蛋白血症组受试患儿的血浆白蛋白水平降低,血浆白蛋白的分布速率(K12)和表观分布容积(Vd)水平升高,半衰期(t1/2)缩短,差异均有统计学意义(P<0.05);低白蛋白血症组受试患儿的免疫球蛋白表达水平低于对照组,患儿的免疫水平降低,差异有统计学意义(P<0.05);低白蛋白血症组患儿的医源性感染发生率高于对照组,差异有统计学意义(P<0.05);相关性分析证明,受试患儿血浆白蛋白浓度与患儿免疫水平呈正相关关系(P<0.05),与医源性感染发生率呈负相关关系(P<0.05)。结论:免疫水平低下以及其导致的医源性感染是发育不良患儿低白蛋白血症的关键致病因素。因此,提高患者免疫力和降低感染发生率可有效预防患儿出现低白蛋白血症的风险。

Abstract: Objective: To explore the correlation between hypoalbuminemia and immune level and iatrogenic infection in children with developmental dysplasia. Methods: A total of 100 cases of developmental dysplasia children with hypoalbuminemia treated in our hospital were selected as the hypoalbuminemia group (n=100) according to their clinical symptoms.While 100 healthy children with similar physical function had medical check-up in our hospital were chosen as the control group (n=100).Their physical condition, plasma albumin level, kinetic parameters plasma albumin metabolism, immunoglobulin expression and the incidence of iatrogenic infection were collected and compared between the two groups.Results:Compared with the control group, the plasma albumin level of the hypoalbuminemia group decreased, while the distribution rate (K12) and apparent distribution volume (Vd) levels increased, and the half-life (t1/2) level decreased, and the differences were statistically significant(P<0.05).The level of immunoglobulin expression in the hypoalbuminemia group was lower than that of the control group, and the immune level of the patients decreased as well, and the differences were statistically significant (P<0.05).The incidence of iatrogenic infection in the hypoalbuminemia group was higher than that in the control group, and the difference was statistically significant (P<0.05).Correlation analysis showed that the serum albumin concentration was positively correlated with the immune level of patients (P<0.05), and negatively correlated with the incidence of iatrogenic infection (P<0.05). Conclusion: The deficiency of immune system and its consequent iatrogenic infection are the key factors for hypoalbuminemia in developmental dysplasia children.Therefore, immune system should be improved and incidence of infections be reduced, which might prevent the children from hypoalbuminemia.
