摘要: 目的:通过对静脉用药集中调配中心(PIVAS)调配环节中出现的各类差错进行分析总结,采取针对性防范措施,提高静脉输液的安全性。方法:统计分析我院PIVAS 2016年1月至2017年12月出现的各类差错记录,同时提出相应的整改和防范措施。结果:2016年1月至2017年12月PIVAS 共调配输液1 670 839袋(瓶),发生差错242次,差错率为0.14‰,主要发生在贴签和加药混合环节。结论:通过有针对性地实施一系列措施可有效降低PIVAS差错率的发生,为临床提供优质的成品输液,有效保障临床用药安全性。

Abstract: Objective:To analyze and summarize errors in the pharmacy intravenous admixture service(PIVAS), and take preventive measures to improve the safety of intravenous infusion. Methods: Statistically analyze error records of PIVAS of a hospital from January 2016 to December 2017, and put forward corresponding corrective and preventive measures. Results: From January 2016 to December 2017, PIVAS deployed 1 670 839 bags(bottles) of infusion, 242 errors occurred, and the error rate was 0.14 ‰, which mainly occurred in the labeling and blending. Conclusion: A series of measures can effectively reduce the error rate of PIVAS, provide high quality finished infusion, and guarantee the safety of clinical medication.
