冠心病患者血浆miR-125b-5p表达变化及其与疾病严重程度关系研究

Changes of Plasma miR-125b-5p Expression in Patients with Coronary Heart Disease and Its Relationship with Severity of Disease

摘要: 目的:探讨冠心病患者血浆miR-125b-5p表达变化及其与疾病严重程度关系。方法:选取我院2014年12月至2017年12月收治的冠心病患者112例为观察组。据临床分型将观察组患者分为急性心肌梗死(acute myocardial infarction,AMI)组,共37例,稳定型心绞痛(stable angina pectoris,SAP)组,共38例,不稳定型心绞痛(unstable angina pectoris,UAP)组,共37例;选取同期正常体检者41例为对照组。检测所有患者血浆miR-125b-5p与高敏C反应蛋白(hs-CRP)表达水平,并分析血浆miR-125b-5p与hs-CRP表达水平与冠心病发病之间的相关性。结果:观察组患者血浆miR-125b-5p水平低于对照组,血浆hs-CRP水平高于对照组(P<0.05)。AMI组患者miR-125b-5p水平低于SAP、UAP、对照组,hs-CRP水平高于SAP、UAP、对照组(P<0.05)。UAP组患者miR-125b-5p水平低于SAP、对照组,hs-CRP水平高于SAP、对照组(P<0.05),SAP患者miR-125b-5p水平低于对照组,hs-CRP水平高于对照组(P<0.05)。经多元回归Logistic分析表示,血浆miR-125b-5p水平低、hs-CRP水平高是冠心病发生的独立危险因素(P<0.05); Pearson相关性分析结果显示:冠心病与hs-CRP呈正相关(P<0.01),与miR-125b-5p呈负相关(P<0.01),miR-125b-5p与hs-CRP呈负相关(P<0.01);miR-125b-5p、hs-CRP对冠心病的诊断价值均较高(P<0.05)。结论:冠心病患者随着疾病越来越严重,其血浆miR-125b-5p水平降低,hs-CRP水平则上升,且两者呈负相关,miR-125b-5p水平低、hs-CRP水平高是影响冠心病的独立危险因素,且两者可作为冠心病的诊断指标。

Abstract: Objective:To investigate the changes of plasma microRNA-125b-5p (miR-125b-5p) expression in patients with coronary heart disease (CHD) and its relationship with the severity of CHD. Methods:112 cases of coronary heart disease admitted to our hospital from December 2014 to December 2017 were selected as the observation group. According to clinical classification, the observation group was divided into acute myocardial infarction (AMI) group, 37 cases, stable angina pectoris (SAP) group, 38 cases, unstable angina pectoris (UAP) group, 37 cases, and 41 cases of healthy people were selected as the control group. The levels of plasma miR-125b-5p and high sensitivity C-reactive protein (hs-CRP) were detected in all patients, and the correlation between the levels of plasma miR-125b-5p and hs-CRP and coronary heart disease was analyzed. Results:The plasma levels of miR-125b-5p in the observation group were lower than those in the control group, and the plasma levels of hs-CRP were higher than those in the control group (P< 0.05). The levels of miR-125b-5p in patients with AMI were lower than those of SAP, UAP and control group, and the levels of hs-CRP were higher than those of SAP, UAP and control group (P< 0.05). The levels of miR-125b-5p in patients with UAP were lower than those of SAP and control group, the levels of hs-CR were higher than those of SAP and control group (P<0.05), the levels of miR-125b-5p in patients with SAP were lower than those of control group, and the levels of hs-CRP were higher than those of control group (P<0.05). Multiple Logistic regression analysis showed that low plasma miR-125b-5p level and high hs-CRP level were independent risk factors for coronary heart disease (P<0.05). The results of Pearson correlation analysis showed that there was a positive correlation between coronary heart disease and hs-CRP (P<0.01), and a negative correlation between coronary heart disease and miR-125b-5p(P<0.01). There was a negative correlation between miR-125b-5p and hs-CRP (P< 0. 01), and the diagnostic value of miR-125b-5p,hs-CRP for coronary heart disease was high (P<0.05).Conclusion:As the disease become more and more serious, the plasma miR-125b-5p level decrease and the hs-CRP level increased in patients with coronary heart disease. There is a negative correlation between the two levels and low miR-125b-5p level, high level of hs-CRP are the independent risk factors of coronary heart disease, and both of them can be used as diagnostic indicators of coronary heart disease.
