萎缩性胃炎患者血清PGⅠ、PGⅡ、IL-2、Hp-IgG抗体、hEGF及miR-129表达水平及其临床意义研究#br#

Expression Levels of Serum PGⅠ, PGⅡ, IL-2, Hp-IgG Antibody,hEGF and miR-129 in Patients with Atrophic Gastritis and Their Clinical Significance#br#

摘要: 目的:探讨慢性萎缩性胃炎患者血清胃蛋白酶原(PG)Ⅰ、PGⅡ、白细胞介素-2(IL-2)、幽门螺杆菌(Hp)-IgG抗体、人表皮生长因子(hEGF)及miR-129表达水平的临床意义。方法:选取2021年1月至2022年12月在我院治疗的慢性萎缩性胃炎患者72例,胃癌患者50例,同时选取健康志愿者50例,检测血清PGⅠ、PGⅡ、IL-2、Hp-IgG抗体、hEGF和miR-129水平。结果:萎缩性胃炎组患者血清PGⅠ、PGⅡ和miR-129水平分别为(70.55±19.42)μg·L-1、(15.71±1.89)μg·L-1和(1.02±0.20),均低于对照组(P<0.05),但均高于胃癌组(P<0.05)。萎缩性胃炎组患者血清IL-2、Hp-IgG抗体阳性率及hEGF水平分别为(15.58±4.32)ng·mL-1、62.50%和(1.49±0.61)ng·mL-1,均高于对照组(P<0.05),但均低于胃癌组(P<0.05)。萎缩性胃炎组Hp-IgG抗体阳性患者血清PGⅠ、PGⅡ和miR-129水平分别为(68.45±14.23)μg·L-1、(14.68±4.03)μg·L-1和(0.99±0.13),均低于Hp-IgG抗体阴性患者(P<0.05),而IL-2和hEGF水平分别为(16.20±2.24)ng·mL-1和(1.61±0.37)ng·mL-1,均高于Hp-IgG抗体阴性患者(P<0.05)。萎缩性胃炎组OLGIM分期Ⅲ~Ⅳ期患者血清PGⅠ、PGⅡ和miR-129水平分别为(67.11±12.62)μg·L-1、(14.20±3.10)μg·L-1和(0.96±0.12),均低于Ⅰ~Ⅱ期患者(P<0.05);OLGIM分期Ⅲ~Ⅳ期患者血清IL-2和hEGF水平分别为(15.90±2.12)ng·mL-1和(1.65±0.34)ng·mL-1,均高于Ⅰ~Ⅱ期患者(P<0.05)。相关性分析显示,萎缩性胃炎组患者血清PGⅠ、PGⅡ水平与hEGF水平呈负相关(r=-0.584和-0.640,P<0.05)。结论: 萎缩性胃炎患者血清PGⅠ、PGⅡ和miR-129水平均降低,IL-2、Hp-IgG抗体及hEGF水平均升高,且血清PGⅠ、PGⅡ水平与hEGF水平存在负相关关系。

Abstract: Objective: To investigate the clinical significance of serum pepsinogens (PGⅠ and PGⅡ), interleukin-2 (IL-2), Helicobacter pylori (Hp)-IgG antibody, human epidermal growth factor (hEGF) and miR-129 levels in patients with chronic atrophic gastritis. Methods: A total of 72 patients with chronic atrophic gastritis and 50 patients with gastric cancer treated in our hospital from January 2021 to December 2022 were selected, and 50 healthy volunteers were selected. The levels of serum PGⅠ, PGⅡ, IL-2, Hp-IgG antibody, hEGF and miR-129 were detected. Results: The serum PGⅠ, PGⅡ and miR-129 levels of patients in chronic atrophic gastritis group were (70.55±19.42) μg·L-1, (15.71±1.89) μg·L-1 and (1.02±0.20) respectively, which were lower than those of the control group (P<0.05), but higher than those of the gastric cancer group (P<0.05). The serum IL-2 level, Hp-IgG antibody positive rate and hEGF levels of patients in the chronic atrophic gastritis group were (15.58 ±4.32) ng·mL-1, 62.50% and (1.49±0.61) ng·mL-1 respectively, which were higher than those of the control group (P<0.05), but lower than those of the gastric cancer group (P<0.05). The serum PGⅠ, PGⅡand miR-129 levels of Hp-IgG positive patients in chronic atrophic gastritis group were (68.45±14.23) μg·L-1, (14.68±4.03) μg·L-1 and (0.99±0.13) respectively, which were lower than those of Hp-IgG negative patients in chronic atrophic gastritis group (P<0.05). The serum levels of IL-2 and hEGF of Hp-IgG positive patients in chronic atrophic gastritis group were (16.20±2.24) ng·mL-1 and (1.61±0.37) ng·mL-1 respectively, which were higher than those of Hp- IgG negative patients in chronic atrophic gastritis group (P<0.05).The levels of serum PG Ⅰ, PGⅡ and miR-129 of OLGIM stage Ⅲ-Ⅳ patients in chronic atrophic gastritis group were (67.11±12.62) μg·L-1, (14.20±3.10) μg·L-1 and (0.96±0.12) respectively, which were lower than those of OLGIM stage Ⅰ-Ⅱ patients (P<0.05). The levels of serum IL-2 and hEGF of OLGIM stage Ⅲ-Ⅳ patients were (15.90±2.12) ng·mL-1 and (1.65±0.34) ng·mL-1 respectively, which were higher than those of OLGIM stage Ⅰ-Ⅱ patients (P<0.05). Correlation analysis showed that serum PGⅠ and PGⅡ levels were negatively correlated with hEGF level in atrophic gastritis group (r=-0.584 and -0.640 respectively, P<0.05). Conclusion: In patients with atrophic gastritis, the serum levels of PGⅠ,PGⅡ and miR-129 decrease, while the serum levels of IL-2, Hp-IgG antibody and hEGF increase, and the serum levels of PGⅠ,PGⅡ were negatively correlated with the level of hEGF.
