2型糖尿病肾病患者肾功能、HcyCys-C水平变化及临床意义

Changes and Clinical Significance of Renal Function Hcy and Cys-C Levels in Patients with Type 2 Diabetic Nephropathy

摘要:

目的:分析2型糖尿病肾病患者肾功能、同型半胱氨酸(Hcy)、胱抑素C Cys-C)水平变化及临床意义。方法:选择我院20231月至20243月诊疗的922型糖尿病患者,按照Mogenson分期分为28例单纯性2型糖尿病患者组、38例早期糖尿病肾病组及26例临床糖尿病肾病组,同期选择35例门诊健康体检者作为健康对照组。比较各组患者肾功能[尿素氮(BUN)、血肌酐(Scr)、内生肌酐清除率(Ccr)、肾小球滤过率(GFR)、尿白蛋白排泄率(UAER)]、HcyCys-C水平,用pearson相关分析糖尿病肾病患者HcyCys-C与肾功能及糖化血红蛋白的相关性,用受试者工作曲线(ROC)分析BUNScrCcrGFRUAERHcyCys-C对糖尿病肾病患者早期肾功能损伤的诊断价值。结果:健康对照组、单纯2型糖尿病组、早期糖尿病肾病组BUNScrCcrGFR比较差异均无统计学意义(P>0.05),临床糖尿病肾病组BUNScr高于健康对照组、单纯2型糖尿病组、早期糖尿病肾病组,CcrGFR低于健康对照组、单纯2型糖尿病组、早期糖尿病肾病组(P<0.05);临床糖尿病肾病组UAERHcyCys-C依次高于早期糖尿病肾病组、单纯2型糖尿病组、健康对照组(P<0.05)。糖尿病肾病患者HcyCys-CBUNScrUAER均呈正相关(P<0.05),HcyCys-CCcrGFR均呈负相关(P<0.05)。ROC分析发现,BUNScrCcrGFRUAERHcyCys-C诊断糖尿病肾病患者早期肾功能损伤的曲线下面积分别为0.7030.7570.7700.8130.8870.8270.844,敏感度分别为57.70%60.12%67.81%70.15%80.80%84.60%88.05%,特异度分别为76.30%77.56%84.20%79.83%81.60%87.32%89.50%HcyCys-C对糖尿病肾病患者早期肾功能损伤诊断的曲线下面积、特异度及敏感度均高于BUNScrCcr。结论:2型糖尿病肾病患者HcyC-ys-C水平上升,能够反映早期肾功能损伤,与2型糖尿病肾病患者的病情程度有关。

Abstract:

Objective: To analyze the changes and clinical significance of renal function homocysteine Hcy and cystatin C Cys-C in patients with type 2 diabetic nephropathy.Methods: 92 patients with type 2 diabetes diagnosed and treated in our hospital from January 2023 to March 2024 were divided into 28 patients with simple type 2 diabetes 38 patients with early diabetic nephropathy and 26 patients with clinical diabetic nephropathy according to Mogenson stage and 35 patients with outpatient health examination were selected as healthy control group during the same period.The levels of renal function blood urea nitrogen BUN), serum creatinine Scr), creatinnine clearance rate Ccr), glomerular filtration rate GFR), urinary albumin excretion rate UAER)], Hcy and Cys-C in patients with diabetic nephropathy were compared and the correlation between Hcy and Cys-C and renal function and glycosylated hemoglobin was analyzed by pearson correlation the diagnostic value of BUN Scr Ccr GFR UAER Hcy and Cys-C in patients with diabetic nephropathy at early stage was analyzed by receiver's work curve ROC.Results: There was no difference in BUN Scr Ccr and GFR among healthy control group simple type 2 diabetes mellitus group and early diabetic nephropathy group P>0.05), BUN and Scr in clinical diabetic nephropathy group were higher than those in healthy control group simple type 2 diabetes mellitus group and early diabetic nephropathy group P<0.05), while Ccr and GFR were lower than those in healthy control group simple type 2 diabetes mellitus group and early diabetic nephropathy group P<0.05); UAER Hcy and Cys-C in clinical diabetic nephropathy group were turnhigher than those in early diabetic nephropathy group simple type 2 diabetes group and healthy control group P<0.05. Hcy and Cys-C were positively correlated with BUN Scr and UAER P<0.05), while Hcy and Cys-C were negatively correlated with Ccr and GFR P<0.05. ROC analysis showed that the area under the curve of BUN Scr Ccr GFR UAER Hcy and Cys-C in the diagnosis of early renal dysfunction in patients with diabetic nephropathy were 0.703 0.757 0.770 0.813 0.887 0.827 0.844 and the sensitivity was 57.70% 60.12% 67.81% 70.15% 80.80% 84.60% 88.05% and the specificity was 76.30% 77.56% 84.20% 79.83% 81.60% 87.32% 89.50% respectively. The ROC area under the curve the specificity and the sensitivity of Hcy and Cys-C were higher than BUN Scr Ccr in the diagnosis of early renal dysfunction in patients with diabetic nephropathy.Conclusion: The elevated levels of Hcy and Cys-C in patients with type 2 diabetic nephropathy can reflect the early renal function injury which is related to the severity of the disease in patients with type 2 diabetic nephropathy.
