数字疗法在慢性阻塞性肺疾病患者中的应用

    基于“互联网+”的延续护理模式对慢阻肺 患者自我管理的研究(D202314019314)

摘要:

目的:探讨数字疗法在慢性阻塞性肺疾病患者中的应用效果。方法:选取20236月至20243月张家界市某三级甲等医院符合纳排标准的40COPD患者作为研究对象,随机分为试验组与对照组,每组各20例。对照组给予常规护理方法,试验组在对照组的基础上,通过数字疗法平台对COPD患者实施远程动态监测,实时监测并反馈患者的用药及运动等情况,并收集患者的健康信息。比较两组患者干预前后改良英国医学研究学会呼吸困难指数(mMRC)、生活质量指数(chronic obstructive pulmonary disease assessment testCAT)评分、肺功能、吸入药物依从性测试(TAI)评分、压力定量气雾剂吸入技能评估、抑郁症筛查量表(PHQ-9)得分、广泛性焦虑自评量表(GAD-7)得分,另外,实验组行软件满意度评分。结果:干预前两组mMRC比较差异无统计学意义(P>0.05),干预后试验组mMRC分级低于对照组(P<0.05);干预前两组患者CAT评分比较差异无统计学意义(P>0.05),干预后试验组CAT评分低于对照组(P<0.05);干预前两组肺功能比较差异无统计学意义(P>0.05),干预后试验组各项肺功能指标明显改善(P<0.05);干预前两组TAI得分比较差异无统计学意义(P>0.05),干预后试验组TAI得分高于对照组(P<0.05);干预前两组压力定量气雾剂吸入技能评估得分比较差异无统计学意义(P>0.05),干预后试验组压力定量气雾剂吸入技能评估得分高于对照组(P<0.05);干预前两组PHQ-9GAD-7得分差异均无统计学意义(P>0.05),干预后两组PHQ-9GAD-7得分差异均无统计学意义(P>0.05)。结论:数字疗法可改善COPD患者的综合情况,但对于焦虑、抑郁的作用还需进一步研究。

Abstract:

Objective: To investigate the effect of digital therapy on chronic obstructive pulmonary disease COPD patients.Methods: A total of 40 COPD patients admitted to a tertiary-level hospital in Zhangjiajie from June 2023 to March 2024 were selected as research subjects and randomly divided into the experimental group and the control group each with 20 cases. The control group received conventional nursing methods while the experimental group in addition to the conventional nursing methods implemented remote dynamic monitoring of COPD patients through the digital therapy platform real-time monitoring and feedback of patients' drug use and exercise and patients' health information was collected. The modified British Medical Research Council dyspnea scale mMRC score quality of life index chronic obstructive pulmonary disease assessment test CAT score pulmonary function the test of the adherence to inhalers TAI scorepressure metered-dose inhaler pMDI inhalation skills assessment depression screening scale PHQ-9), and generalized anxiety disorder self-assessment scale GAD-7 were compared before and after intervention in both groups. In addition the software satisfaction score was collected in the experimental group.Results: The results showed that there was no significant difference in mMRC scores between the two groups before intervention P>0.05); and the mMRC grading of the experimental group was lower than that of the control group after intervention P<0.05), there was no significant difference in CAT scores between the two groups before intervention P>0.05), and CAT scores of the experimental group was lower than that of the control group after intervention P<0.05. There was no significant difference in pulmonary function between the two groups before intervention P>0.05), and pulmonary function of the experimental group had improved more than that of the control group after intervention P<0.05. There was no significant difference in TAI scores between the two groups before intervention P>0.05), and TAI scores of the experimental group was higher than that of the control group after intervention P<0.05. There was no significant difference in pressure metered dose inhaler pMDI skills assessment scores between the two groups before intervention P>0.05), and pMDI skills assessment scores of the experimental group was higher than that of the control group after intervention P<0.05. There was no significant difference in PHQ-9 and GAD-7 scores between the two groups before intervention and after intervention P>0.05.Conclusion: Digital therapeutics can improve the overall condition of COPD patients but the effect on anxiety and depression needs further research.
