基于孟德尔随机化的癌症与老年性黄斑变性因果关系探讨

Causal Relationship Between Cancer and Age-Related Macular Degeneration Based on Mendelian Randomization

摘要:

目的:观察性研究显示老年性黄斑变性(AMD)与血管生成活跃型恶性肿瘤存在潜在联系,但由于观察性研究可能受混杂因素、反向因果关联及偏倚干扰,其结论难以建立可靠的因果推断。本研究采用双样本孟德尔随机化(MR)方法,基于遗传变异作为工具变量(IVs),探究AMD与肿瘤之间的因果关系,为开发针对AMD及相关肿瘤的药物提供理论依据。方法:本研究采用逆方差加权(IVW)、MR-Egger、加权中位数、加权模型和简单模式5MR分析方法评估AMD与癌症风险的关系。结果:在对AMD和癌症的MR分析中,IVW结果显示,肺癌(OR = 1.04395% CI 1.009~1.078P = 0.014)、肝癌(OR = 0.99095% CI 0.920~1 066P = 0.798)、宫颈癌(OR = 1.00095% CI 1.000~1.000P = 0.703)、黑色素瘤(OR = 1.00095% CI 1.000~1.001P = 0.106)、甲状腺癌(OR = 0.98195% CI0.910~1.058P = 0.620)、结直肠癌(OR = 0.99195% CI0.966~1.016P = 0.476)、乳腺癌(OR = 0.99995% CI0.999~1.000P = 0.004)、胃癌(OR = 0.98695% CI0.953~1.020P = 0.410)。结论:AMD是肺癌的危险因素(OR>1P = 0.014),是乳腺癌的保护因素(OR<1P = 0.004)。AMD与肝癌、宫颈癌、黑色素瘤、甲状腺癌、结直肠癌、胃癌之间无明显因果关系(P>0.05)。

Abstract:

Objective: Observational studies suggest a potential association between age-related macular degeneration AMD and angiogenically active malignant tumors. However observational studies are susceptible to confounding factors reverse causality and bias making it difficult to establish reliable causal inferences. This study propose to use a two-sample Mendelian randomization MR method to investigate the causal relationship between AMD and cancers based on genetic variation as an instrumental variable IVs), and to provide a theoretical base for the development of drugs targeting AMD and related cancers.Methods: This study employ five MR analysis methodsInverse Variance Weighted IVW), MR-Egger Weighted Median Weighted Model and Simple Mode to assess the potential causal link between AMD and cancer risk.Results: In MR analysis of AMD and cancerthe IVW results showed that lung cancer OR = 1.043 95% CI 1.009-1.078P = 0.014), hepatocellular carcinoma OR = 0.990 95% CI 0.920-1 066P = 0.798), cervical cancer OR = 1.000 95% CI 1.000-1.000P = 0.703), malignant melanoma OR = 1.000 95% CI 1.000-1.001P = 0.106), thyroid cancer OR = 0.981 95% CI0.910-1.058P = 0.620), colorectal cancer OR = 0.991 95% CI 0.966-1.016P = 0. 476), breast cancer OR = 0.999 95% CI 0.999-1.000P = 0. 004), gastric cancer OR = 0.986 95% CI 0.953-1.020P = 0.410.Conclusion: AMD was a risk factor for lung cancer OR > 1 P = 0.014 and a protective factor for breast cancer OR < 1P = 0.004. There was no significant causal relationship between AMD and hepatocellular carcinoma cervical cancer malignant melanoma thyroid cancer colorectal cancer and gastric cancer P > 0.05.
