不同类型眩晕患者血清同型半胱氨酸及维生素B12、叶酸表达变化分析

Changes of Serum Homocysteine and Vitamin B12,Folic Acid Levels in Patients with Different Types of Vertigo

摘要: 目的:探讨眩晕患者血清同型半胱氨酸(homocysteine,Hcy)、维生素B12、叶酸及血尿酸(serum uric acid,UA)表达变化,分析Hcy、维生素B12、叶酸及UA水平与不同类型眩晕相关性。方法:选取2015年4月至2017年4月我院神经内科收治的眩晕患者126例为观察组,按病因不同选择,良性阵发性位置性眩晕(BPPV)38例,后循环缺血(PCI)41例,前庭性偏头痛(VM)21例,持续性姿势-知觉性眩晕/头晕(PPPD)26例,同期选取正常体检者65例为对照组。观察比较两组患者及不同类型眩晕患者Hcy、维生素B12、叶酸及UA表达变化,并分析导致眩晕的独立危险因素。结果:观察组患者Hcy、UA水平高于对照组,维生素B12、叶酸水平低于对照组(P<0.05)。PCI、PPPD患者的Hcy、UA水平高于VM、BPPV患者,维生素B12、叶酸水平低于VM、BPPV患者(P<0.05)。经多因素Logistic回归分析可得,Hcy、UA是影响眩晕患者的独立危险因素(P<0.05)。结论:Hcy、UA水平升高与眩晕密切相关,是导致眩晕患者发病的独立危险因素,不同类型眩晕患者Hcy、UA水平有所不同,PCI及PPPD与之更为相关。

Abstract: Objective:To investigate the changes of serum homocysteine(Hcy), vitamin B12, folic acid and serum uric acid (UA)levels in vertigo patients and their relationship with different types of vertigo.Methods:126 patients with vertigo admitted to the department of neurology of our hospital from April 2015 to April 2017 were selected as the observation group, according to the etiology, there were 38 cases of benign paroxysmal positional vertigo(BPPV), 41 cases of posterior circulation ischemia(PCI), 21 cases of vestibular migraine(VM) and 26 cases of persisten postural-perceptual dizziness(PPPD), 65 healthy persons were selected as the control group.The changes in the expression of Hcy, vitamin B12, folic acid and UA in the two groups of patients and different types of vertigo were compared and the independent risk factors leading to vertigo were analyzed.Results:The levels of Hcy and UA in the observation group were higher than those in the control group, and the levels of vitamin B12 and folic acid were lower than those in the control group (P<0.05).The levels of Hcy and UA in patients with PCI and PPPD were higher than those in patients with VM and BPPV, while the levels of vitamin B12 and folic acid were lower than those in patients with VM and BPPV (P<0.05).Multivariate Logistic regression analysis showed that Hcy and UA were independent risk factors for vertigo patients(P<0.05).Conclusion:The elevated levels of Hcy and UA are closely related to vertigo, and are independent risk factors for vertigo. The levels of Hcy and UA are different in patients with different types of vertigo, PCI and PPPD are more related.
