建中和胃汤治疗脾虚畏寒型慢性浅表性胃炎的疗效及对血清炎性因子的影响

摘要： 目的：观察自拟建中和胃汤治疗脾虚畏寒型慢性浅表性胃炎的疗效及对患者血清炎性因子表达水平的影响。方法：选取本院2016年6月至2018年6月间收治的104例脾虚畏寒型慢性浅表性胃炎患者作为研究对象，按照随机数字表法对入选患者进行分组，对照组、观察组分别纳入52例，对照组给予常规西药治疗，观察组在对照组的基础上联合应用自拟建中和胃汤治疗；治疗一疗程（60 d）后，评估两组临床疗效，观察两组患者治疗前后中医证候积分、血清胃肠激素、血清炎性因子的变化情况，统计治疗前后两组幽门螺旋杆菌（Hp）阳性率，比较两组治疗前后血清胃肠激素和炎性因子水平。结果：治疗后，两组患者胃脘痞满、胃脘隐痛、嗳气、大便异常、肢冷畏寒、纳差积分均较前降低（P<0.05），且观察组均低于对照组（P<0.05）；治疗一疗程后，观察组临床总有效率为92.31%，对照组为76.92%，观察组临床疗效优于对照组（P<0.05）；一疗程后，两组患者血清中肿瘤坏死因子-α（TNF-α）、胃泌素（GAS）、白细胞介素-6（IL-6）、胃动素（MTL）表达均优于本组治疗前（P<0.05），且观察组均优于对照组（P<0.05）；治疗后，对照组Hp阳性率为26.92%，观察组为11.54%，两组均较治疗前降低（P<0.05），且观察组Hp阳性率低于对照组（P<0.05）。结论：建中和胃汤治疗脾虚畏寒型慢性浅表性胃炎疗效理想，能够提高Hp根除率，改善胃动力，抑制炎性因子过高表达。

Abstract: Objective：To observe the efficacy of self-proposed Jianzhong hewei decoction in the treatment of chronic superficial gastritis due to spleen deficiency and chillness and its effect on the level of serum inflammatory factors in patients. Methods： 104 cases of chronic superficial gastritis with spleen deficiency and chillness admitted to our hospital from June 2016 to June 2018 were selected as the objects of this study.The selected patients were divided into the control group and the observation group according to the random number table， each group 52 cases.The control group was treated with common western medicine， and the observation group was treated with self-proposed Jianzhong hewei decoction on the basis of the control group.After one course of treatment (60 d)， the clinical efficacy of the two groups was evaluated， and the changes of TCM syndrome scores， serum gastrointestinal hormones and serum inflammatory factors in the two groups before and after treatment were observed， and the positive rate of helicobacter pylori (Hp) in the two groups before and after treatment were calculated. Level of serum inflamatory factors in the two groups were compared. Results： After treatment， the scores of epigastric ruffian fullness， epigastric pain， belching， abnormal feces， limb coldness and chillness， and inappetence in the two groups were all lower than those before treatment (P<0.05)， and the scores in the observation group were all lower than those in the control group(P<0.05).After one course of treatment， the total effective rate was 92.31% in the observation group and 76.92% in the control group. The overall level of clinical efficacy and the total effective rate in the observation group were both better than those in the control group(P<0.05).After treatment， the serum levels of TNF-α，GAS， IL-6， MTL in the patients of the two groups were improved than before treatment (P<0.05)， and the observation group was superior to the control group (P<0.05). After treatment， the Hp positive rate was 26.92% in the control group and 11.54% in the observation group， both were lower than that before treatment (P<0.05)， and the Hp positive rate in the observation group was lower than that in the control group (P<0.05). Conclusion：Jianzhong hewei decoction has ideal curative effect in the treatment of chronic superficial gastritis due to spleen deficiency and chillness， which can improve the eradication rate of Hp， improve gastric motility and inhibit the overexpression of inflammatory factors.
