64排CT低管电压下低剂量对比剂联合生理盐水CTPA在肺动脉栓塞中的应用研究

摘要: 目的:探讨64排CT低管电压下低剂量对比剂联合生理盐水肺动脉CT造影(CTPA)在肺动脉栓塞(PE)中的应用价值。方法:将76例PE可疑病例随机分为对照组和观察组各38例并行CTPA,对照组管电压120 kVp、60 mL对比剂,观察组管电压80 kVp、40 mL对比剂,并联合生理盐水20 mL。其余扫描参数相同。分别比较两组诊断的灵敏度、特异度、阳性预测值、阴性预测值及诊断准确率,图像的主、客观质量以及辐射剂量。结果:两组诊断的灵敏度、特异度、阳性预测值、阴性预测值及诊断准确率差异均无统计学意义(P>0.05);图像主观质量比较,观察组优良率高于对照组(P<0.05),两组合格率差异无统计学意义(P>0.05);客观质量比较,观察组图像噪声高于对照组(P<0.05),其余质量指标均高于对照组; 观察组辐射剂量低于对照组(P<0.05)。结论:64排CT低管电压低剂量对比剂联合生理盐水CTPA对PE诊断具有重要价值,图像质量满意,在保证诊断准确率的同时可降低辐射剂量,减少对比剂用量。

Abstract: Objective:To explore the application value of 64-row CT with low-tube voltage and low-dose contrast agent combined with normal saline CT pulmonary angiography(CTPA) in pulmonary embolism (PE). Methods: 76 cases suspected of PE were divided into control group and observative group randomly with 38 cases in each group and accepted CTPA all. In control group,the contrast agent was 60 mL and the tube voltage was 120 kVp, while in observative group, the contrast agent was 40 mL and the tube voltage was 80 kVp and combined with 20 mL of normal saline. The rest scanning parameters are the same.The sensitivity, specificity, positive predictive value, negative predictive value, accuracy of diagnosis and subjective quality, objective quality and radiation dosage of 2 groups were compared respectively. Results: There were no significant differences in the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of diagnosis between 2 groups (P>0.05). The comparison of subjective image quality showed the superior rate of observative group was higher than that control group (P<0.05), and the qualified rate was without difference between 2 groups (P>0.05). The objective image quality comparison showed the image noise of observative group was higher than that of control group (P<0.05), and the rest items were superior to control group (P<0.05). The radiation dose of observative group was significantly lower than that of control group (P<0.05). Conclusion: CTPA of 64-row CT with low-tube voltage and low-dose contrast agent combined with normal saline show important value in the diagnosis of PE, satisfactory image quality and higher diagnostic accuracy can be obtained while the radiation dose and the dosage of contrast agent can be reduced.
