摘要： 目的:探讨Narcotrend应用于全凭静脉麻醉联合喉罩插入在脑功能区术中唤醒麻醉的价值及效果。方法:50例颅脑急诊手术患者分为观察组和对照组,每组25例,都采用异丙酚与瑞芬太尼全凭静脉联合喉罩插入麻醉,在整个手术过程中所有患者接受Narcotrend指数监测,对照组根据临床经验来调节麻醉,观察组依据Narcotrend指数值调整麻醉。结果:观察组术前、术中唤醒前、术中唤醒即刻和术毕的平均动脉压和心率变化均较对照组低且平稳(P<0.05)。经过观察与统计,观察组的术中唤醒时间与拔管时间都明显少于对照组(P<0.05)。结论:Narcotrend指数调控下全凭静脉麻醉使颅脑功能区手术患者血流动力学更平稳,术中唤醒时间与拔管时间都更快,有利于脑功能保护,值得推广应用。

Abstract: Objective:To investigate the experiences and value of narcotrend apply to total intravenous anesthesia combined with laryngeal mask insertion in the brain area intraoperative wake-up anesthesia.Methods:50 cases of brain emergency surgery were equally divided into groups and the control group,all patient were given propofol and remifentanil total intravenous anesthesia combined laryngeal mask insertion,all patients throughout the procedure were accepted narcotrend index monitoring,the the control group based on clinical experience to adjust the anesthesia,Narcotrend index value based on the observation group to adjust anesthesia.Results:The preoperative,intraoperative wake-up before,during and immediately wake-up surgery in mean arterial pressure and heart rate low and stable compared with the control group(P<0.05).Through observation and statistics,intraoperative observation group wake-up time and extubation time were significantly less than the control group(P<0.05).Conclusion:Narcotrend index under the control of total intravenous anesthesia to patients undergoing brain functional areas more stable hemodynamics,intraoperative wake-up time and faster extubation time,help to protect brain function,should be widely applied.
