畲族人群双侧上肢收缩压差异与动脉硬化研究

Study on Inter Arm Difference of Systolic Blood Pressure of Bilateral Upper Limbs and Arteriosclerosis in the She Nationality

摘要: 【摘要】目的:探讨中国福建畲族自然人群双侧上肢收缩压差异(IAD)与动脉硬化的关系。方法:2009年入选畲族自然人群418例,其双侧上肢血压能够通过动脉硬化自动检测装置(VS-1000)同时记录得到。IAD为左侧与右侧收缩压差值的绝对值,分别按IAD不同水平分成两组,即IAD10组(IAD<10mmHg和IAD≥10mmHg),IAD15组(IAD<15mmHg和IAD≥15mmHg组)。结果:在IAD增高组(IAD≥10mmHg组或IAD≥15mmHg),其年龄、男性比例、颈-股动脉脉搏波传导速度(CF-PWV)水平显著高于IAD正常组(IAD<10mmHg或IAD<15mmHg组),双侧踝臂指数(ABI)水平显著低于IAD正常组。将IAD作为连续性变量进行分析,提示年龄、超敏C反应蛋白(hs-CRP)、白细胞计数(WBC)、CF-PWV与IAD呈显著正相关性(r=0.125、0.155、0.104、0.164,P<0.05),双侧ABI与IAD呈显著负相关性(r=-0.297[左]、-0.309[右],均P<0.01)。进一步控制了年龄和性别之后,hs-CRP(r=0.150,P=0.021)、CF-PWV(r=0.198,P=0.002)、双侧ABI(r左=-0.411,r右=-0.350,均P=0.000)仍与IAD显著相关。结论:在中国福建畲族自然人群中,IAD增高者的动脉硬化水平显著高于IAD正常者,ABI显著低于IAD正常者。此外,ABI与IAD显著负相关,CF-PWV、WBC、hs-CRP与IAD显著正相关。

Abstract: 【ABSTRACT】Objective: To investigate the relationship between bilateral upper extremity difference in systolic blood pressure and atherosclerosis in She Nationality natural population of Fujian in China. Methods: 418 cases of the She Nationality natural population in 2009 were enrolled into the study. The bilateral upper extremity blood pressure were recorded simultaneously by arteriosclerosis device(VS-1000) automatically. Inter arm difference(IAD) was computed as the absolute value of left arm blood pressure minus right arm blood pressure. According to the different levels of IAD, we divided all subjects into two groups, namely group IAD10 (IAD<10mmHg and IAD≥10mmHg), IAD15 group (IAD<15mmHg and IAD≥15mmHg). Results: In IAD increased group(IAD≥10mmHg or IAD≥15mmHg), age, male proportion, carotid femoral pulse wave velocity (CF-PWV) were significantly higher than that in normal IAD group (IAD<10mmHg or IAD<15mmHg), bilateral ankle brachial index(ABI) was significantly lower than that of normal IAD group. Analysis using IAD as a continuous variable showed that age, high sensitive C reactive protein(hs-CRP), white blood cell count(WBC), and CF-PWV were significantly positive correlation with IAD(r=0.125, 0.155, 0.104, 0.164, P<0.05), there was a significant negative correlation between IAD and bilateral ABI(r=-0.297[left] and -0.309[right]. P<0.01). After controlling for age and sex, hs-CRP (r=0.150, P=0.021), CF-PWV (r=0.198, P=0.002), bilateral ABI(r left =-0.411, r right =-0.350, P=0.000, P=0.021) were still significantly correlated with IAD. Conclusion: In natural population of Fujian She Nationality in China, the level of arteriosclerosis was significantly higher and ABI was lower in higher IAD group than in normal IAD group. In addition, ABI and IAD were significantly negatively correlated, CF-PWV, WBC, hs-CRP and IAD were significantly positively correlated.
