血脂异常患者HDL-C、甲状腺激素与冠心病的相关性分析

Correlation Analysis between HDL-C、Thyroid Hormone Levels and Coronary Heart Disease in Patients with Dyslipidemia

摘要: 目的:研究长期规范降脂治疗的血脂异常人群高密度脂蛋白胆固醇(HDL-C)、促甲状腺素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平并分析HDL-C、TSH、FT3、FT4与冠心病(CHD)的相关性。方法:收集2014年4月至2017年4月于我院心血管内科住院经冠状动脉造影明确诊断为冠心病的血脂异常患者共152例作为冠心病组,另外收集同期我院心内科住院排除了冠心病的血脂异常人群105例作为对照组。入院后分别收集两组血清样本行血脂水平和甲状腺功能检测。结果:冠心病组和对照组的HDL-C、TSH、FT3、FT4水平比较,HDL-C、FT3差异有统计学意义(P<0.05);冠心病组及对照组TSH、FT3、FT4与 HDL-C均无明显相关(P>0.05);在对照组中,TSH、FT4与TG, TSH与总胆固醇(TC)有相关性,差异有统计学意义(P<0.05)。结果:长期规范降脂治疗的冠心病患者HDL-C、FT3水平均下降,提示HDL-C可能减少、延缓冠心病的发生和发展,而甲减可能是冠心病的危险因素之一。

Abstract: Objective:To investigate the level of HDL-C、TSH、FT3、FT4 in patients with dyslipidemia who have long-term lipid-lowering therapy and the correlation between HDL-C、TSH、FT3、FT4 and coronary heart disease(CHD). Methods: We collected 152 cases of dyslipidemia patient diagnosed with CHD by coronary angiography in our hospital from April 2014 to April 2017 as CHD group and collected 105 cases of dyslipidemia patient diagnosed without CHD as control group. The serum samples in two groups were collected to detect the serum lipid levels and thyroid function. Results: Compared with the control group,the differences of HDL-C and FT3 from CHD group were statistically significant(P<0.05). However, in CHD group and control group, there was no significant correlation(P>0.05)between TSH、FT3、FT4 and HDL-C. In control group,TSH、FT4 correlated with TG,TSH correlated with TC,the difference was statistically significant(P<0.05).Conclusion: The level of HDL-C and FT3 in patients with dyslipidemia and CHD decreased,which point out that HDL-C may defer the development of coronary heart disease and hypothyroidism is one of the risk factor.
