摘要

目的:探究芪杞益肾胶囊通过NLRP3/GSDMD途径抑制足细胞焦亡,延缓糖尿病肾病进展的作用。方法:建立糖尿病肾病大鼠模型,将48SD大鼠随机分为空白组、模型组、贝那普利阳性对照组、芪杞益肾胶囊治疗组,每组各12只。检测大鼠尿液24 h尿蛋白定量,血清中血尿氮素(BUN)、血肌酐(SCr)、甘油三酯(TG)、总胆固醇(TC)水平等;ELISA法检测IL-IL-18;观察肾脏病理形态学;免疫组化法检测大鼠肾组织GSDMD蛋白表达;蛋白免疫印迹法(Western blot)检测肾组织NLRP3Caspase-1Nephrin蛋白表达。结果:与空白组相比,模型组大鼠尿蛋白、血脂(TGTC)、BUNSCr升高,病理上出现肾小球部分毛细血管袢肥大,系膜基质明显增多,基底膜增厚,同时IL-IL-18水平升高,肾组织中GSDMD表达增加,NLRP3Caspase-1表达增加,足细胞标志蛋白Nephrin表达减少(P<0.05)。与模型组相比,治疗组尿蛋白、血脂、BUNSCr均出现不同程度下降,肾组织中系膜基质减少,基底膜增厚减轻,足突呈部分融合,IL-IL-18水平降低,肾组织中GSDMDNLRP3Caspase-1表达降低,Nephrin蛋白水平恢复。结论:芪杞益肾胶囊可能通过调控NLRP3/GSDMD通路,抑制足细胞焦亡,降低尿蛋白,延缓肾脏病进展。

Abstract

Objective: To investigate the effect of Qiqi Yishen capsules on inhibiting podocyte pyroptosis through NLRP3/ GSDMD pathway and delaying the progression of diabetic nephropathy.Methods: By establishing diabetes nephropathy DN rat model 48 SD rats were randomly divided into five groups with 12 rats in each group which were the blank group the model group benazepril positive control group Qiqi Yishen capsule treatment group. Compare and analyze the levels of 24-hour urinary protein blood urine nitrogen BUN), blood creatinine SCr), triglyceride TG) ,total cholesterol TC of the rats and renal pathological morphology was observed. IL-1β and IL-18 were detected by ELISA.The expression of GSDMD protein in rat kidney was detected by immunohistochemistry. The expressions of NLRP3 Caspase-1and Nephrin in renal tissues were detected by Western blot.Results: Compared with the blank group the levels of urinary protein blood lipid TG TC)、 BUN SCr increased in the model group. Meanwhile there was capillary loop hypertrophy of glomerular tissue in renal tissue. Mesangial matrix increased significantly and basement membrane thickened. The levels of IL-1β and IL-18 and the expressions of GSDMD NLRP3 and Caspase-1 increased and the expression of podocyte marker Nephrin decreasesd P<0.01. Compared with the model group urine protein blood lipid BUN SCr decreased in the treatment group. At the same time mesangial matrix decreased. The levels of IL-1β and IL-18 decreased. The expressions of GSDMD NLRP3 and Caspase-1 decreased and the level of Nephrin protein recovered in the treatment groups.Conclusion: Qiqi yishen capsules may inhibit podocyte pyroptosis reduce proteinuria and delay the progression of kidney disease by regulating NLRP3/GSDMD pathway.
