Analysis of Image Features of Adenomyosis Patients Based on Time-intensity Curve of Contrast-enhanced Ultrasound

摘要: 目的:通过超声造影检测所得时间-强度曲线(TIC)分析子宫腺肌症患者的图像特点。方法:将2017年3月至2018年6月于我院妇科初诊提示子宫腺肌症且经手术病理学检测证实患者分为局灶型组(n=44)和弥漫型组(n=68),同时选择同期于我院门诊体检正常的育龄期妇女52例作为对照组。经腹部或阴道进行超声造影检测,通过时间-强度曲线分析子宫腺肌症患者的图像特点。结果:弥漫型组和局灶型组患者增强方式、增强均匀度及增强水平与对照组比较,差异均有统计学意义(P<0.05)。两组患者上升时间(RT)均短于对照组、达峰时间(TTP)均快于对照组、峰值强度(IMAX)均高于对照组,差异均有统计学意义(P<0.05)。结论:子宫腺肌症患者超声造影检测可见向心性、不均匀高增强,RT短于正常人群、TTP快于正常人群、IMAX高于正常人群,上述特征均为子宫腺肌症的重要诊断依据。

Abstract: Objective: To analyze the image characteristics of patients with adenomyosis by the time-intensity curve(TIC) obtained by contrast-enhanced ultrasound. Methods: From March 2017 to June 2018, patients for the first gynecological examination in our hospital prompted adenomyosis and confirmed by surgical pathology were divided into the limited group(n=44) and the diffuse group (n=68). In the same period, 52 women of childbearing age who were normal in our outpatient clinic were choosed as the control group. Ultrasound angiography was performed through the abdomen or vagina, and the image characteristics of patients with adenomyosis were analyzed by time-intensity curve. Results: Compared with the control group the enhancement mode, uniformity and enhancement level in the diffuse group and the limited group were different, and the difference was statistically significant(P<0.05). The rise time (RT) of the two groups was shorter than that of the control group, the peak time (TTP) was faster than that of the control group, the peak intensity (IMAX) was higher than that of the control group, the difference was statistically significant(P<0.05).Conclusion: Contrast-enhanced ultrasonography in patients with adenomyosis can be seen centripetal, non-uniformly high, shorter RT than normal, faster TTP than normal, and higher IMAX than normal. These characteristics are the important diagnostic criteria for adenomyosis.
