Abstract: Objective： To evaluate the effectiveness of the hospital-community-individual diabetes integrated management model. Methods： 258 patients with diabetes mellitus admitted in our hospital from January to December 2017 were selected as subjects，and it was divided into the control group (n=128) and the experimental group (n=130) according to the random number table method. Patients the control group were adopted routine management model，and patients in the experimental group were adopted the hospital-community-personal diabetes integrated management model. There was comparison of diabetes management effects between the two groups patients.Results： The self-management ability score and patient satisfaction score of the experimental group were higher than the control group(P<0.05). The glycosylated hemoglobin（HbA1c）， fasting blood glucose（FBG） level and 2h postprandial blood glucose（PBG） level in the experimental group were lower than the control group(P<0.05). The incidence of complications in the experimental group (2.34%) was lower than that in the control group (10.77%)， the difference was statistically significant(P<0.05). Conclusion： In the treatment of diabetic patients， hospital- community- individual diabetes integrated management can effectively control blood sugar and HbA1c levels， improve patient self-management ability and patient satifaction degree， and clinical promotion and application value is high.
