清热利湿法预防阿帕替尼引起的蛋白尿的临床研究

Clinical Study on Prevention of Proteinuria Caused by Apatinib through Clearing Heat and Removing Dampness

摘要: 目的:探讨清热利湿法在预防阿帕替尼引起的蛋白尿中的作用。方法:选取2016年3月至2018年1月我院肿瘤科收治的胃癌晚期化疗失败患者80例,观察组和对照组各40例。对照组单独口服阿帕替尼250 mg, 每天1次,服用12周;观察组在对照组治疗的基础上口服自制中药汤剂,疗程为12周。观察两组患者治疗前及治疗后24 h尿蛋白定量、肌酐、尿素氮、血清胱抑素C的水平,比较12周后两组疗效。结果:治疗12周后,观察组24 h尿蛋白定量、血清胱抑素C指标较对照组更低,差异有统计学意义(P<0.01),肌酐、尿素氮两组差异无统计学意义(P>0.05);两组12周后疗效比较差异无统计学意义(P>0.05)。结论:清热利湿法在预防、减轻阿帕替尼引起的蛋白尿上发挥了优势,从而保证阿帕替尼的临床疗效。

Abstract: Objective:To investigate the effect of prevention proteinuria caused by apatinib through clearing heat and removing dampness method. Methods: From March 2016 to January 2018, 80 patients with advanced stomach carcinoma chemotherapy failure in the department of oncology of our hospital were randomly divided into the observation group and the control group, with 40 cases in each group. The control group received oral apatinib 250 mg once a day for 12 weeks; the observation group on the basis of the treatment of the control group, took self-made traditional Chinese medicine decoction for 12 weeks. The levels of 24 h urinary protein, creatinine, urea nitrogen and serum cystatin C were observed before and after treatment. The efficacy after 12 weeks was compared between the two groups. Results: After 12 weeks of treatment, the levels of 24 h urinary protein and serum cystatin C of the observation group were lower than those of the control group, the difference was statistically significant (P<0.01), and the difference of creatinine and urea nitrogen between the two groups was not statistically significant (P>0.05). There was no statistical difference in efficacy after 12 weeks of treatment between the two groups (P>0.05). Conclusion: This study shows that the method of clearing heat and removing dampness plays an important role in preventing and alleviating proteinuria caused by apatinib, thus ensuring the clinical efficacy of apatinib.
