摘要: 目的:探讨重症急性胰腺炎(severe acute pancreatitis,SAP)患者出现多部位感染时抗菌药物的优化。方法: 以临床药师参与治疗1例SAP患者发生的胆道感染及医院获得性肺炎(hospital acquired pneumonia,HAP)合并尿路感染(urinary tract infection,UTI)为例,结合多个指南及文献,分析存在感染的多部位的可能致病菌、感染相关的危险因素,兼顾多部位感染的情况,优化抗菌药物方案。结果: 临床药师的优化方案控制了患者的多部位感染。结论: 通过该病例分析,为今后在临床中患者出现多部位感染时优化抗菌药物方案提供参考。

Abstract: Objective: To investigate the optimization of antibacterial therapy in patients with severe acute pancreatitis with multi-site infection. Methods: Taking a case of clinical pharmacist involved in the treatment of biliary tract infections of SAP patients with hospital-acquired pneumonia combined with urinary tract infection as an example, combined with multiple guidelines and literature, and analyzed the possible pathogens in multiple parts of the infection and the risk factors related to the infection, the clinical pharmacist optimized the antibacterial therapy in consideration of the condition of multiple sites. Results: Optimized protocols supposed by clinical pharmacists controled the multi-sites infection of the patients. Conclusion: This case analysis can provice reference for optimizing antibacterial therapy for patients with multi-sites infections in the future.
