肿块型和管周浸润型肝内胆管细胞癌的CT影像学征象分析

摘要: 的:探究肝内胆管细胞癌(intrahepatic cholangiocarcinoma,ICC)不同分型的CT影像学征象。方法:收集我院2017年6月至2019年12月期间收治的经手术病理确诊为ICC患者37例,回顾性分析研究不同分型ICC的CT影像学特征。结果:本研究所涉37例患者,男性23名(62.2%),女性14名(37.8%),年龄范围43~68岁,平均年龄(55.7±7.2)岁,按照肿瘤生长方式分为肿块型ICC和管周浸润型ICC。其中肿块型ICC共计29例(78.4%),位于肝左叶18例,肝右叶11例,肿块直径3.0~11.5 cm,病灶形态不规则。CT影像学特征为肝包膜收缩征、鱼网状、延迟强化征及胆管扩张征。管周浸润型ICC患者共计8例(21.6%),均位于肝左叶,呈树枝状或长条状,CT平扫示多见肝左叶外侧段萎缩,动态增强后呈渐进性中度强化,管壁向心性增厚、管腔狭窄。结论:掌握肿块型ICC和管周浸润型ICC患者CT影像学的特征性改变有助于临床诊断。

Abstract: Objective: To explore the CT imaging features of different types of intrahepatic cholangiocarcinoma (ICC). Methods:37 patients with ICC confirmed by pathologic diagnosis after surgery were enrolled, who were admitted in our hospital from June 2017 to December 2019. The CT imaging features of different types of ICC were analyzed retrospectively. Results: In this study, there were 23 males (62.2%) and 14 females(37.8%) in 37 involved patients,and the age range were from 43 to 68 years old, and the average age was (55.7±7.2) years. According to the growth mode, tumors were divided into the mass type and periductal infiltrating type. 29(78.4%) cases were mass type, of which 18 cases were located in the left hepatic lobe, and 11 cases were located in the right liver lobe, with diameter of 3.0-11.5 cm. The tumor shape was irregular. The CT imaging features of mass type were hepatic capsule contraction, fish mesh, delayed enhancement and bile duct dilatation. 8 cases (21.6%) were periductal infiltration type, which were located in the left hepatic lobe, presenting as dendritic or long strips. CT scanning showed the atrophy of the lateral section of left hepatic lobe, and dynamic contrast enhanced CT showed the progressive moderate enhancement of the thickening tube and luminal stenosis. Conclusion:CT images features of mass type and periductal infiltrating type of ICC have characteristic changes, which are helpful for clinical diagnosis.
