癫痫患儿丙戊酸钠不良反应及影响因素分析

摘要: 目的:探究癫痫患儿服用丙戊酸钠不良反应发生情况及影响因素,为临床合理用药提供参考。方法:收集本院108例癫痫患儿的基本信息、用药情况、肝肾功能指标以及丙戊酸钠不良反应情况并测定其血药浓度,探究丙戊酸钠对肝肾功能的影响,采用Logistic回归方法分析其不良反应发生相关因素。结果:肝功能指标谷丙转氨酶、谷草转氨酶和γ-谷氨酰转肽酶水平以及肾功能指标尿酸和肌酐水平在服药后较服药前增高(P<0.05);服药后<3岁患儿的谷丙转氨酶、谷草转氨酶水平高于≥3岁患儿(P<0.05),而白蛋白、总蛋白、尿素氮和肌酐水平则低于≥3岁患儿(P<0.05)。多因素Logistic回归分析得到年龄[OR=0.203, 95%CI(0.075, 0.552), P=0.001]、用药种数[OR=4.816, 95%CI(1.745, 13.289), P=0.002]和血药浓度[OR=8.456, 95%CI(2.419, 29.554), P=0.000]是癫痫患儿丙戊酸钠不良反应发生的独立影响因素。结论:癫痫患儿服用丙戊酸钠存在肝肾功能的损伤,年龄影响肝肾功能损伤的程度。<3岁、采用联合用药方案以及血药浓度≥100 μg·mL-1的癫痫患儿更容易发生不良反应,需加强用药监护,保证用药安全。

Abstract: Objective: To investigate the adverse drug reactions (ADRs) induced by valproate (VPA) in children with epilepsy and analyze the relevant influencing factors to provide reference for rational medication. Methods: The clinical data and ADRs of VPA were collected from 108 children with epilepsy in our hospital, and their VPA serum concentration were measured. Explore the effect of VPA on liver and renal function and analyze the influencing factors of ADRs. Results: The levels of ALT, AST, γ-GT, UA and Cr were higher after VPA administration (P<0.05). The levels of ALT and AST were higher in children<3 years old after VPA administration (P<0.05), while ALB, TP, BUN and Cr levels were lower in children <3 years old (P<0.05). Multivariate Logistic regression analysis showed that age[OR=0.203, 95%CI(0.075, 0.552), P=0.001], drug combination [OR=4.816, 95%CI(1.745, 13.289), P=0.002] and VPA serum concentration[OR=8.456, 95%CI(2.419, 29.554), P=0.000] were independent risk factors of ADRs of VPA. Conclusion: VPA results in liver dysfunctions and renal impairment in children, and age affects the degree of liver and renal function damage. Younger children with polytherapy, drug combination and high VPA serum concentration are more likely to occur ADRs. It is necessary to pay close attention to medication monitoring and ensure medication safety.
