日间手术模式开展结直肠癌根治手术的可行性与影响因素分析

摘要： 目的：探讨日间手术模式开展结直肠癌根治手术的可行性与影响因素。方法：回顾性分析2018年1月至2019年12月我院消化外科同一医疗组收治的74例采用“当日住院，当日手术”模式管理的结直肠癌根治手术患者的临床病理资料，分析影响48 h 内出院的独立危险因素。结果： 74例结直肠癌手术病例中，46例（62.2%）48 h完成入院、手术及出院。Charlson评分、N分期是结直肠癌根治手术病例48 h 内出院的独立预测因素。结论：在规范开展高质量手术的前提下，结合加速康复外科理念的实施，Charlson评分低、N分期早的结直肠癌手术病例48 h 内完成入院、手术与出院的可能性更大。

Abstract: Objective： To explore the feasibility and influencing factors of radical resection for colorectal cancer(RRCRC) based on ambulatory surgery mode. Methods： The clinicopathological data of 74 cases of patients undergoing RRCRC between January 2018 and December 2019 managed by "operation when admission" mode were retrospectively collected. Logistic regression analyses were performed to determine the significant risk factors for assessing the likelihood of discharge within 48 h. Results： Among 74 patients undergoing RRCRC in the study， 46 patients(62.2%) were admitted， operated and discharged within 48 h. The multivariable logistic regression analysis indicated that Charlson index， N staging were independently associated with the likelihood of discharge within 48 h. Conclusion： Under the premise of standardized high-quality surgery， combined with the implementation of enhanced rehabilitation after surgery， patients undergoing RRCRC with lower Charlson index and early N stage are more likely to complete admission， surgery and discharge within 48 h.
