摘要: 的: 探讨基于阶段转变理论(TTM)的社区健康教育对老年2型糖尿病(T2DM)患者自我管理行为的影响。方法:以随机数字表法将120例老年T2DM患者分为实验组(n=60,在TTM指导下由全科医生团队成员进行的健康教育)和对照组(n=60,开展常规健康教育),两组均干预12个月。比较两组患者干预前后血糖水平、自我管理行为和自我效能变化。结果:干预后两组患者空腹血糖(FBG)、餐后2 h血糖(2hPBG)水平均低于干预前,实验组低于对照组(P<0.05)。干预后两组患者自我管理行为量表(SDSCA)及糖尿病管理自我效能量表(C-DMSES)各维度评分均高于干预前,实验组高于对照组(P<0.05)。结论:基于TTM的社区健康教育可有效地增强老年T2DM患者糖尿病自我管理行为能力及患者自我效能,对血糖的控制效果更理想。

Abstract: Objective: To explore the influence of community health education based on the transtheoretical model (TTM) on self-management behavior of elderly patients with type 2 diabetes mellitus (T2DM). Methods: According to the random number table, 120 elderly patients with T2DM were divided into the experiment group (n=60), in which the health education was conducted by members of the general practitioner team under the guidance of TTM, and the control group (n=60), in which the regular health education was conducted. Both groups were intervened for 12 months. The changes of blood glucose levels, self-management behavior (SDSCA) and self-efficacy (C-DMSES) before and after the intervention were compared between the two groups. Results: The levels of FBG and 2hPBG after the intervention in the two groups were lower than those before the intervention, and the experiment group was lower than the control group (P<0.05). The scores of SDSCA and C-DMSES after the intervention in the two groups were higher than those before the intervention, and the experiment group was higher than the control group (P<0.05). Conclusion: Community health education based on TTM can effectively enhance the diabetes self-management behavior and self-efficacy of elderly patients with T2DM, and has good effect on blood sugar control.
