经尿道前列腺等离子剜除术与电切术治疗前列腺增生的效果比较

摘要/Abstract

摘要： 目的：比较经尿道前列腺等离子剜除术与电切术治疗前列腺增生的效果并探讨其临床意义。方法：回顾性分析2018年1月至2019年12月我院收治的124例前列腺增生患者临床资料，按照其接受的治疗方式分为等离子剜除组（TUKEP，n=60例）和电切组（TURP，n=64例），比较两组患者的术前情况，评价两种手术方式的疗效。结果：两组患者的年龄、身体质量指数（body mass index，BMI）、术前最大尿流率（maximum urine flow rate，Qmax）、术前残余尿量（volume of residual urine，PVR）、术前国际前列腺症状评分（international prostatic symptom score，IPSS）的差异均无统计学意义(P＞0.05)，等离子剜除组的手术时间、术中出血量、术后冲洗时间均少于电切组（P＜0.05）；两组患者的术后Qmax、术后IPSS评分、尿失禁发生率的差异均无统计学意义(P＞0.05)。结论：经尿道前列腺等离子剜除术与电切术均能达到较好的治疗前列腺增生的效果，但剜除术因手术时间短、出血少、术后恢复快而更具优势。

Abstract: Objective： To explore the effect of transurethra plasmakinetic enucleation of prostate（TUKEP） and transurethra resection of prostate（TURP） in the treatment of benign prostatic hyperplasia (BPH) and its clinical significance. Methods： The clinical data of 124 patients with BPH from January 2018 to December 2019 in our hospital were analyzed retrospectively. According to therapy methods， all patients were divided into TUKEP group (n=60 cases) and TURP group (n=64 cases). The preoperative conditions of patients in the two groups were compared， and the curative effect of the two surgical methods was evaluated. Results： There were no statistical significant differences in the age， body mass index (BMI)， maximum urine flow rate (Qmax)， volume of residual urine (PVR)， and international prostatic symptom score (IPSS) before surgery in the two groups. The operation time， blood loss during operation and postopretive bladder washing time in the TUKEP group were shorter than those in the TURP group ( P＜0.05). There was no significant difference in Qmas， IPSS and incontinence between the two groups(P＞0.05). Conclusion： Both TUKEP and TURP can achieve good therapeutic effect in the treatment of BPH， however， TUKEP has more advantages due to its short operation time， less bleeding and quicker recovery after surgery.
