Abstract: Objective： By understanding the situation of medical insurance payment refusal of our hospital in 2018 and 2019， to analyze the reasons for refusal， and propose solutions. Methods： Datas on medical insurance payment refusal of drug costs in our hospital in 2018 and 2019 from the medical records of patients participating in medical insurance were colected， and the reasons for payment refusal were analyzed. Results： The number of drug varieties refused payment by medical insurance in our hospital decreased in 2019 compared with that in 2018， and the amount of refusal was basically the same. Most of the drugs with high refusal amount were auxiliary drugs， including key monitoring drugs， traditional Chinese medicine injections， antibacterial drugs， etc. Conclusion：The hospital should establish an effective and feasible supervision mode of rational drug use for key drug varieties， effectively reduce the incidence of irrational drug use， promote clinical rational drug use， and reduce the rate of medical insurance payment refusal.
