七氟烷对全身麻醉手术患者免疫功能及应激反应的影响

Effects of Sevoflurane on Immune Function and Stress Response in Patients Undergoing General Anesthesia Surgery

摘要: 目的:探讨七氟烷对全身麻醉手术患者免疫功能及应激反应的影响。方法:以我院收治的80例全身麻醉手术患者为研究对象,随机分为对照组(n=40)和观察组(n=40),选例时间为2020年1月至2020年12月。对照组采用丙泊酚复合瑞芬太尼全身麻醉,观察组采用七氟烷复合瑞芬太尼全身麻醉。两组患者均于术后观察3 d。比较两组患者围术期指标、T0~T3免疫功能及应激指标水平。结果:相较于对照组,观察组患者术后自主呼吸恢复时间、苏醒时间、拔管时间、肛门排气时间均较短(P<0.05);与T0比,T1~T3两组患者CD4+、CD4+/CD8+、血清超氧化物歧化酶(SOD)、过氧化氢酶(CAT)、谷胱甘肽过氧化物酶(GSH-PX)水平均呈先降低、后升高的趋势,且相较于对照组,观察组患者处于更高水平(P<0.05);两组患者血清去甲肾上腺素(NE)水平呈先升高、后降低趋势,且相较于对照组,观察组患者处于更低水平(P<0.05)。结论:全身麻醉手术患者应用七氟烷可有效缩短术后恢复时间,保护免疫系统功能,缓解氧化应激反应。

Abstract: Objective: To investigate the effect of sevoflurane on immune function and stress response in patients undergoing general anesthesia surgery. Methods: A total of 80 patients undergoing general anesthesia in our hospital from January 2020 to December 2020 were selected and divided into the control group (n=40) and the observation group (n=40) according to a random number table. The control group was given general anesthesia with propofol combined with remifentanil, and the observation group was given general anesthesia with sevoflurane combined with remifentanil. Both groups were observed 3 d postoperatively. The perioperative indexes, T0-T4 immune function and the levels of stress indexes were compared between the two groups. Results: Compared with the control group, the postoperative spontaneous breathing recovery time, wake-up time, extubation time, and anal exhaust time in patients of the observation group were shorter than those of the control group(P<0.05). Compared with T0, the CD4+, CD4+/CD8+, serum SOD, CAT, GSH-PX levels of of the two groups in T1-T3 of patients first decreased and then increased, and the observation group was higher than the control group(P<0.05). The serum NE level of the two groups of patients increased first and then decreased, and the observation group was lower than the control group (P<0.05). Conclusion: Sevoflurane can effectively shorten the recovery time postoperative, protect the immune system function and alleviate oxidative stress response in patients undergoing general anesthesia.
