临床药师开展肿瘤患者药学服务的实践

摘要/Abstract

摘要： 目的：探讨临床药师为肿瘤患者提供药学服务的切入点及工作体会。方法：通过分析临床药师对肿瘤患者实施药品不良反应监测、调整抗感染方案、制定个体化癌痛治疗方案及干预重点监控药品合理使用的典型病例，总结药学服务实践经验体会。结果：临床药师根据肿瘤患者特点找到不同切入点开展药学服务，与医生形成治疗团队，提高了临床治疗效果。结论：临床实践中，临床药师可从药品不良反应监测、个体化给药方案制定及重点监控药品合理使用等方面入手开展肿瘤患者药学服务。临床药师深入临床才能及时发现并解决药物治疗相关问题，在诊疗活动中发挥专业价值。

Abstract: Objective： To discuss the entry point and working experience of clinical pharmacist providing pharmaceutical care for cancer patients. Methods： Through analyzing the typical cases of clinical pharmacist carrying out adverse drug reaction monitoring， adjusting anti-infection regimens， formulating individualized cancer pain treatment regimen and promoting the rational use of key monitoring drugs， to summarize the experiences of pharmaceutical care. Results： According to the characteristics of cancer patients， clinical pharmacist found different entry points to carry out pharmaceutical care， formed the treatment team with doctors， and improved the clinical treatment effect. Conclusion： In clinical practice, Clinical pharmacists can provide pharmaceutical care for cancer patients through monitoring adverse drug reaction, formulating individualized medication regimen and promoting the rational use of key monitoring drugs. Clinical pharmacists participating in the clinical practice can discover and solve problems related to drug treatment in time and play the professional value in clinical activities.
