髓芯减压联合植骨术治疗股骨头坏死的研究进展

摘要: 股骨头坏死是骨科临床中最常见且难治的疾病之一。可由创伤或非创伤因素导致,且具有较高的致残率。该病进展较快,但由于初期症状不显著,较易忽视或误诊,从而使病情快速发展,使患者活动受限甚至功能丧失,从而严重影响患者生活质量。因此,有效治疗股骨头坏死,阻止或减缓其疾病进展是骨科医生所面临的一大挑战。目前临床上治疗股骨头坏死主要有保守治疗和手术治疗,对于股骨头坏死中期患者,保守治疗难以阻止塌陷进程,更多用于缓解症状,因此手术治疗成为这一阶段治疗首选。髓芯减压联合植骨术以其良好的疗效而被广泛关注,多用于早中期股骨头坏死的治疗。本文就髓芯减压联合植骨术治疗股骨头坏死的作用机制和临床疗效作一综述,以期为临床治疗该疾病提供参考。

Abstract: One of the most common and difficult to treat diseases in orthopaedic practice is osteonecrosis of the femoral head (ONFH). It can be caused by traumatic or non-traumatic factors and has a high disability rate. ONFH progresses rapidly, but because the initial symptoms are not significant, it is easy to be ignored or misdiagnosed, which lead to rapid progression, limiting the patient′s activities and even loss of function, thus seriously affecting the patient′s quality of life. Therefore, it is a major challenge for orthopedic surgeons to effectively treat ONFH and stop or slow down its progression. At present, the clinical treatment of ONFH mainly includes conservative treatment and surgical treatment. For patients in the middle stage of ONFH, conservative treatment is difficult to stop the collapse process, more for symptom relief, so surgery has become the first choice for this stage of treatment. Core decompression combined with bone grafting has been widely used for the treatment of early to mid-stage ONFH because of its good efficacy. This article reviews the mechanism of action and clinical efficacy of core decompression combined with bone grafting in the treatment of ONFH to provide a reference for the clinical treatment of ONFH.
