吡嗪酰胺相关高尿酸血症的影响因素分析

Analysis of Influencing Factors of Hyperuricemia Induced by Pyrazinamide

摘要: 目的:研究吡嗪酰胺用于治疗结核相关疾病引起高尿酸血症的影响因素。方法: 采用回顾性研究,利用医院信息系统(HIS)调取咸阳市第一人民医院2016年1月1日至2020年12月31日使用吡嗪酰胺片患者的病历资料,利用单因素分析及Logistic回归模型进行多因素分析。结果:纳入病例510例,298例发生了高尿酸血症,发生率为58.43%。单因素分析结果显示,发生高尿酸血症组与未发生高尿酸血症组的性别、年龄、体重和用药前血尿酸水平的差异均有统计学意义(P<0.05)。Logistic多因素回归分析显示,男性较女性发生高尿酸血症的风险高(OR=1.827, 95%CI:1.235~2.704,P<0.05);<60岁的患者较≥60岁的患者易发生高尿酸血症(OR=0.512, 95%CI:0.346~0.758,P<0.05);用药前血尿酸水平282~420 μmol·L-1较用药前血尿酸水平<282 μmol·L-1的患者更易发生高尿酸血症(OR=4.126,95% CI:2.465~6.908,P<0.05)。结论:性别、年龄、用药前血尿酸水平是吡嗪酰胺相关高尿酸血症的影响因素。

Abstract: Objective:To study the influencing factors of hyperuricemia caused by pyrazinamide in the treatment of tuberculosis. Methods:A retrospective study was conducted by obtaining the case data of patients using pyrazinamide in the First People′s Hospital of Xianyang from January 1, 2016 to December 31, 2020 through hospital information system(HIS),and single factor analysis and logistic regression model for multivariate analysis were used.Results:Among the 510 induded cases, 298 cases (58.43%) occurred hyperuricemia. Univariate analysis showed that there were significant differences(P<0.05) in gender, age,weight and serum uric acid level between the hyperuricemia group and the non-hyperuricemia group.Logistic multivariate regression analysis showed that men had a higher risk of hyperuricemia than women, which was statistically significant(OR=1.827,95%CI:1.235-2.704,P<0.05). Patients aged<60 years old were more prone to hyperuricemia than patients aged ≥60 years old, which was statistically significant(OR=0.512, 95%CI:0.346-0.758,P<0.05).Patients with serum uric acid level of 282~420 μmol·L-1 before treatment were more likely to develop hyperuricemia than those with serum uric acid level of<282 μmol·L-1before treatment(OR=4.126,95%CI:2.465-6.908, P<0.05).Conclusion:Gender, age and value of serum uric acid before medication were influencing factors of hyperuricemia induced by pyrazinamide.
