心肌酶谱与cTnI、BNP联合检测在冠心病患者危险分层及手术效果的评估价值

The Value of Combined Detection of Myocardial Enzymes, cTnI and BNP in Evaluating Risk Stratification and Predicting Surgical Results of Coronary Heart Disease

摘要: 目的:探讨心肌酶谱与肌钙蛋白I(cTnI)、脑钠肽(BNP)联合检测在冠心病(CHD)危险分层及手术效果的评估价值。方法:将我院收治的120例CHD患者作为研究对象,其中急性心肌梗死(AMI)、心绞痛(AP)患者均为60例,另选取同期于我院体检的60例健康志愿者作为对照组,所有纳入者均进行心肌酶谱[乳酸脱氢酶(LDH)、谷草转氨酶(AST)、肌酸磷酸激酶(CK)]与cTnI、BNP检测,对比不同危险分层患者的心肌酶谱与cTnI、BNP水平,并分析心肌酶谱与cTnI、BNP联合检测对冠状动脉搭桥手术效果的预测价值。结果:CHD组患者血清LDH、AST、CK、cTnI、BNP水平均高于对照组(P<0.05),AMI组血清LDH、AST、CK、cTnI、BNP水平高于AP组(P<0.05)。CHD组患者血清LDH、AST、CK、cTnI、BNP水平:重度组>中度组>轻度组(P<0.05)。预后不良组血清LDH、AST、CK、cTnI、BNP水平均高于预后良好组(P<0.05)。心肌酶谱与cTnI、BNP水平联合检测评估冠脉血管病变严重程度的AUC=0.922(0.871~0.993),特异度91.57%、灵敏度86.84%、准确度90.08%。心肌酶谱与cTnI、BNP水平联合检测评估手术预后效果的AUC=0.872(0.768~0.976),特异度89.62%、灵敏度85.71%、准确度89.17%。结论:心肌酶谱与cTnI、BNP联合检测对CHD危险分层和手术效果具有良好评估价值,可以反映CHD病变程度及预后效果。

Abstract: Objective:To explore the value of combined detection of myocardial enzymes, cardiac troponin I (cTnI) and brain natriuretic peptide (BNP) in evaluating risk stratification and predicting surgical results of coronary heart disease (CHD). Methods:120 patients with CHD admitted to our hospital were taken as the research subjects, including 60 cases of acute myocardial infarction (AMI) and 60 cases of angina pectoris (AP). Another 60 healthy volunteers who received physical examination in our hospital during the same period were selected as the control group.The levels of myocardial enzymes [lactate dehydrogenase (LDH), aspartate aminotransferase (AST), creatine phosphokinase (CK)], cTnI and BNP were detected among all the included subjects, and were compared between different risk stratifications. The predictive value of combined detection of myocardial enzymes, cTnI and BNP on surgical results was analyzed. Results:The levels of serum LDH, AST, CK, cTnI and BNP of patients in the CHD group were higher than those in the control group(P<0.05). The levels of serum LDH, AST, CK, cTnI and BNP of patients in the AMI group were higher than those in the AP group (P<0.05). Comparison of levels of serum LDH, AST, CK, cTnI and BNP of CHD patients showed that severe group>moderate group>mild group (P<0.05).The levels of serum LDH, AST, CK, cTnI and BNP of patients in the poor prognosis group were higher than those in the good prognosis group (P<0.05). The AUC specificity, sensitivity and accuracy combined detection of myocardial enzymes, cTnI and BNP in evaluating the severity of coronary artery disease were 0.922(0.871~0.993), 91.57%, 86.84% and 90.08% respectively.The AUC specificity and sensitivity and accuracy of combined detection of myocardial enzymes, cTnI and BNP in predicting surgical results were 0.872(0.768~0.976), 89.62%, 85.71% and 89.17% respectively.Conclusion: Combined detection of myocardial enzymes, cTnI and BNP has good value in evaluating risk stratification and predicting surgical results of CHD, and can reflect the degree of CHD lesions and the treatment effect.
